Unsurprisingly, Medicare spending exploded in 2020, with an increase of 6.5%, split between city care (+ 3.6%), medical establishments (+ 9%) and establishments medico-social (+ 10.2%). Within city care, the reimbursement structure has been profoundly modified by the health crisis, with four main phases. From January to February 2020, before the crisis, reimbursements remained in line with 2019. Then, from March to May 2020, the first confinement resulted in a decrease in consultations with doctors, not offset by the sharp increase teleconsultations. At the same time, the daily allowances (DA), paid in the event of a work stoppage, have increased sharply due to the derogatory measures.

The summer period, from June to August, marked a return to normal, but with significant disparities: partial recovery of care for some (specialists, dentists), offset by a still low activity for others (face-to-face consultations for doctors, medical transport). Finally, the period from September to December saw a sharp increase in RT-PCR tests, antigenic tests, the issue of masks and the acceleration of daily allowances.

In the end, over the year as a whole, reimbursements for medical and dental care fell by 6.4%, while reimbursements for medical biology jumped 45.7% (after + 1.8% in 2019, – 0.5% in 2018). The total amount of expenditure for Covid screening in medical biology laboratories would thus amount to 1.8 billion euros in 2020. Similarly, daily allowances exploded by 25% in 2020 (after + 4.9 % in 2019 and 2018 and + 4.6% in 2017). The dynamic was extremely upset, with a doubling in April-May, then a decline, before accelerating during the resumption of the epidemic with an increase of 15% and 20% over the last four months of the year.
