“In 2015, I made the observation of my medical demography and the average age of the inhabitants”, recalls André Accary (various right), president of the departmental council of 71. At this moment, he “crosses the curves” … and quickly notices that he is heading “straight into the wall”. “Afterwards, you accept or not the fatality”, he sums up.
André Accary then takes his pilgrim’s staff. “I carried out two years of investigation in the field. I went to see the universities, the deans, the liberals, the retirees, the students…” A solution emerges: the salaried system.
Non-competition
“Beyond the salaried workforce, I wanted to build a real health project,” insists the president of 71. “Initially, he did not have the blessing of the French State, because I did not have the competence to recruit doctors to consult. I was against all odds on this device. Which gives it even more charm, “he smiles. And one of the guidelines of the project is non-competition. “The installation of my doctors is done in total coherence with the liberal fabric”, affirms André Accary. “When a liberal doctor moves in, I withdraw.”
Six sites, spread throughout the Saône-et-Loire, exist today. With 60 general practitioners “with very varied profiles, from 28 to 67 years old, with 48% men and 52% women”.
For “those who stay”
Everyone wins, assures the president of the Department. The patients, of course. But also “those who remain”. “We are reducing the pressure on liberal doctors. When, in a small town of 3,000 inhabitants, you have three doctors and two leave, the one who remains takes a headache. He cannot accept 2,500 patients from more. He then finds himself under constant pressure … “
Cost: 2 M € of investment, in a budget annexed to that of the departmental council (more than 650 M € in total). “At the start, there is a risk-taking,” concedes the president. “The goal is for the business to balance itself out. It will take a little while.” Salaries paid to doctors follow “the hospital grid”.
