Evolution of the epidemiological situation

The Minister of Health estimated on Saturday March 21 the number of real cases between 30,000 and 90,000 on the national territory.

Tuesday March 24

An unprecedented, severe and rapid epidemic according to J. Salomon

On Tuesday March 24, Professor Salomon estimated the number of deaths from Covid-19 at 1100 since the start of the epidemic in the hospital sector, including 240 in yesterday alone. 85% were over 70 years old.
– 22,302 cases confirmed by PCR tests
– More than 10,170 patients have been hospitalized to date, including 2,516 in serious condition and in intensive care. 34% of patients in intensive care are under 60 years of age and 58% between 60 and 80 years of age.
– 3,281 were able to recover from Covid-19 and were discharged from hospital. 12,000 people did not require hospitalization.
– The number of intensive care beds has increased from 5,000 to 8,000 in France
– 9,000 tests are performed per day, with a gradual increase to more than 10,000 tests per day by the weekend, then to more than 10,000 tests from next week

The HCSP issued an opinion on March 23: VSSARS-CoV-2 oronavirus:therapeutic recommendations

100,000 dead in Ehpad?
According to the Director General of Health, the 1,100 deaths in the hospital sector “represent only a small part of the mortality” on the national territory. “The two main places of death are the hospital and nursing homes,” he said before announcing the implementation of daily mortality monitoring in institutions for the elderly.

A few days ago the National Professional Geriatrics Council wrote a letter to the Minister of Health in which it fears the death of 100,000 patients in nursing homes and retirement homes and requests 500,000 masks per day for these establishments.

The law establishing a state of health emergency for two months has come into effect.
President Macron’s Scientific Council is pushing for a containment of at least 6 weeks.

Monday March 23

186 new deaths of COVID-19 positive patients were reported at the hospital on Monday. The total of deaths thus passes to 860.

3,176 new cases confirmed by PCR bring the total number of COVID cases 19 to 19856.
1,435 new hospitalizations in 24 hours. 8,670 people hospitalized in total on Monday, of which 2,000 are resuscitated (+335 in the last 24 hours)

Sunday March 22

The number of confirmed cases has now reached 16,018, compared to 14,459 the previous day.

7,240 patients are hospitalized for COVID-19 this Sunday. 1,746 serious cases are in intensive care. 674 deaths on the national territory, including 112 on yesterday.

In Italy, 650 deaths are to be deplored during the day. The worst is yet to come in Spain.

Saturday March 21

The Minister of Health estimates the number of real cases between 30,000 and 90,000 on the national territory

This Saturday, March 21, 2020, there are 14,459 cases confirmed by PCR test. Hospital surveillance identifies 6,172 people hospitalized, including 1,525 serious cases, in intensive care. 562 people died in hospital. Severe forms are observed even in young adults: 50% of people hospitalized in intensive care are less than 60 years old.

According to the DGS, we are rapidly evolving towards a generalized epidemic on the territory. Many uncertainties remain as much about the behavior of the virus as about the impact of individual and collective barrier measures (strict confinement, respect for the distance of one meter between people), which are still imperfectly applied. One thing is certain, serious cases and deaths can be avoided by being very respectful of the instructions.

Containment measures are in place for a minimum of 15 days. The extension of containment will depend on the observance of the application of containment by the population and the evolution of the epidemic situation.

Currently, more than 4,000 tests are performed each day and the capacities are not saturated. Over 60,000 tests have already been carried out.

The transition to the epidemic phase has led to an evolution in screening conditions, in order to favor the management of the most serious cases. People who are fragile, at risk, hospitalized, residents of collective accommodation of people at risk and health professionals, new homes or territories are systematically tested.

We must all strictly observe the confinement instructions, the barrier measures (very regular hand washing) and the minimum distance of one meter between people on the street, in queues, in transport, at work, reduce massively our contacts throughout the day.

Doctors should prioritize remote care as much as possible, in the interest of patients who are homebound and can call their doctor, and to protect caregivers. As of March 21 and throughout the duration of the epidemic, telemedicine and teleconsultation are fully supported.

Monday March 16

A total of 6,633 confirmed cases and 148 deaths have been reported

A new package to limit the spread of the epidemic is announced by President Macron:

· Travel will be limited from March 17 to what is strictly necessary with financial penalties for offenders

· The borders of the Schengen area are closed and travel between non-European countries and the European Union will be suspended

· A new mask distribution policy has been introduced.

· A minimum daycare service for the caregivers’ children is implemented.

In the evening of March 15, 923 new cases for a total of 5,423. 36 new deaths were reported, bringing the total to 127. 400 people are hospitalized in serious condition.

With the first cases recorded in Ariège and Indre, there is no longer a French department without positive cases for COVID-19.

For Pr SALOMON, the situation is very worrying. He stressed in the morning on France Inter the importance of respecting barrier measures such as hand washing and also social distancing in order to stem the epidemic.

The French must understand that we can all spread the virus. Many still do not follow the instructions.

Without collective awareness, the French healthcare system could quickly reach saturation, forcing healthcare professionals to make dramatic choices. He adds that each French person must decrease by 3 or 4 the number of people he approaches.

On the evening of March 14, 4,500 confirmed, i.e. 839 additional cases during the day and 20 additional deaths, which brings the total number of deaths to 91. 300 serious cases are hospitalized

Stage 3 is triggered by the executive. The number of cases doubles every 72 hours. The curve follows that of the Italian contamination figures. Some hospitals are starting to reach their limit when it comes to intensive care beds.

All public places not essential to life must be closed on March 15 at midnight. The French are invited to stay with as much as possible and limit their travel.

An inmate from Fresne is contaminated.

On the evening of March 13, France had 785 additional confirmed cases of Covid-19, which brings the total number of cases to 3661. 21 new deaths are to be deplored for a total of 79.

First case recorded in Guadeloupe, in the hollow and in the cantal.

Two cases are reported in the French prisons of Fresnes and Poitiers: it is a nurse and a doctor.

Gatherings of more than 50 people are prohibited

In France, this Thursday, March 12, 2020, according to the latest available data, there are 2,876 cases confirmed by a PCR type test, 595 more than the day before; 129 serious cases requiring resuscitation have been identified; 61 people died, 12 more than yesterday. The virus is now actively circulating in several areas of the national territory, the epidemic is increasing in France.

9 COVID-19 outbreaks have been identified: in the agglomeration of Montpellier, in Aude, Calvados, in Corsica, in Haute-Savoie, in Oise, Morbihan, Haut-Rhin, to which are added the group of tourists returning from a trip to Egypt.

President Macron in a television address announced new measures to slow the epidemic. Nurseries, schools and universities closed from Monday March 16 until further notice. Measures are in preparation to ensure the care of the children of health professionals. Jean-Michel Blanquer spoke of a “class at home” mechanism.

If public transport is maintained, the French are invited to “limit their trips to what is strictly necessary”. New measures are planned to limit gatherings.

In hospitals, non-essential care will be postponed and the head of state calls for “immediate deprogramming of non-emergency surgical procedures” in order to free up beds in intensive care, recovery rooms and qualified staff – in accordance with stage 2 of the white plan of hospitals.

It encourages as many doctors and carers as possible, including students and young retirees, to mobilize on behalf of the hospitals and the health reserve.

“The government will mobilize whatever means necessary, whatever the cost, to provide assistance and save lives,” said Macron.

The World Health Organization now describes the global situation of COVID-19 as a pandemic, that is to say, confirms the worldwide spread of the epidemic.

On the evening of March 11, Olivier Véran announced a total of 2,281 diagnosed cases, or 497 more in twenty-four hours. Covid-19 killed 48 people, 25 men and 23 women, 15 more than the day before. 105 patients are in intensive care.

Two new homes have appeared: Corsica and 19 towns in eastern Montpellier, where nurseries and schools will be closed for fourteen days, and gatherings of more than 50 people banned.

In addition to the eight households already identified in Calvados, Aude, Corsica, Haute-Savoie, Oise, Morbihan, Haut-Rhin, two new clusters are added in Corsica and east of Montpellier where 19 municipalities where nurseries and schools will be closed for 14 days, and gatherings of more than 50 people prohibited.

Tuesday March 10, 3 new deaths and 372 additional positive cases were announced by the DGS. The number of cumulative cases was 1,784 and the number of deaths was 33.

Of the 33 deaths, there are no minors. 23 were over 75 and 5 were under 64. Most cases of death are accompanied by factors of co-morbidities.

86 patients are to date in serious condition and placed in intensive care.

Stage 3 inevitable but not yet pronounced

Professor SALOMON recalled that:
– 98% of patients with Covid-19 recover
– “80% to 85% of the forms remain benign”,
– “there are still many scientific uncertainties about this virus” unlike that of the flu, which kills 10,000 a year.

“It is not a flu, el COVID-19 can give serious forms on people not that old” Pr Salomon.

According to Pr Salomon’s press conference, 9 new deaths and 286 new cases linked to Covid-19 were listed on Monday March 9 by Public Health France. This brings the number of deaths to 30 and 1,412 confirmed cases.

France is currently in stage 2 of the management of COVID-19 Coronavirus infection. To date, the virus is present in certain territories and circulates, even if it is not present everywhere in the territory.

In the Oise and Haut-Rhin regions, the virus is circulating and progressing with great intensity, stage 2 measures have been reinforced and the health care system has been adapted.

The priority is to slow the circulation and spread of the virus on the territory. To do this, the simplest and most effective measure is to apply all barrier gestures. These actions will help reduce the increase in the number of cases, the transmission of the virus and reduce the epidemic peak.

It is recommended to avoid visits to the elderly, especially those of children under 15 or people with symptoms or sick. In health establishments, visits are limited to one person per patient. Minors and sick people should not visit hospitalized people, including maternity.

300,000 students deprived of school in France, 300 million worldwide

All schools are closed in the departments of Oise and Haut-Rhin. Dozens of nurseries, schools, colleges, high schools, medical and educational institutions and university sites are also affected by temporary closings in Corsica, the Paris region, or the south. A total of 300,000 children are deprived of school.

Culture Minister Franck Riester was infected as were five deputies and two agents of the National Assembly.

As of March 8, the number of confirmed cases has crossed the symbolic bar of 1,000 to position itself up to 1,126 confirmed cases, including 117 new cases the day of March 8, 2020. France is the 5th country most affected by this virus. With 3 new deaths, the number of deaths linked to the SARS-CoV-2 coronavirus is now 19.

Protests of more than 1000 people banned

After the defense council held on Sunday evening in the presence of President Macron, Olivier Véran announced that rallies of more than 1,000 people are now prohibited until further notice. The minister asked the prefects to put together a list of rallies whose usefulness for the nation could exempt them from prohibitions such as transport, demonstrations, competitions.

So far, closed gatherings of more than 1,000 people have been banned at least until April 15, according to an order issued on Saturday.

These increased restrictions are expected to have a serious impact on the world of sport and entertainment.

Maintaining stage 2, the transition to stage 3 inexorable

The minister refuses for the moment to announce stage 3 of the Orsan REB plan even if President Macron announced that his passage was inexorable. Stage 3 confirms the epidemic or pandemic situation. It is no longer a question of stemming the circulation of the virus, but of mitigating the epidemic by fully mobilizing the entire health system to avoid saturation of hospital capacities.

Concretely, it will be a question for the liberal health professionals of ensuring an assumption of responsibility in ambulatory of the least serious cases whereas the 38 authorized establishments will have to focus above all on the most serious cases.

Flexible telemedicine and uncapped overtime

The minister also announced that overtime hours will be cut for hospital staff.

Finally, the Minister encourages the French to consult remotely as much as possible using telemedicine systems. In order to be reimbursed by social security, it will no longer necessarily be necessary to go through your doctor.

I decided to lift the obligation to pass [en amont] by his attending physician (…) before carrying out a remote consultation“, Indicated the minister who should sign a decree on Monday to this effect.
