A breast self-examination campaign was recently shut down due to concerns. This week the Dutch Breast Cancer Association will start a new campaign. This is important, because how do you actually do that, check your breasts?
De actie kwam in maart online, maar werd offline gehaald na kritiek van het het Nederlands Huisartsengenootschap (NHG).
De strekking van de campagne was dat vrouwen periodiek – iedere derde dag van de maand – hun borsten zouden moeten onderzoeken. Het idee was dat iedere derde dag van de maand een goed geheugensteuntje zou zijn.
Maar daarbij werd volgens het NHG geen rekening gehouden met het feit dat veel vrouwen in bepaalde delen van hun menstruatiecyclus last hebben van knobbeliger borstklierweefsel. Een gevolg daarvan is dat vrouwen sneller iets kunnen voelen zonder dat er iets aan de hand hoeft te zijn.
Bovendien is niet aangetoond dat borstkanker eerder wordt opgespoord en gediagnosticeerd dankzij regelmatig zelfonderzoek.
De ‘regel van drie’ blijft belangrijk
Borstkankervereniging Nederland-directeur Cristina Guerrero Paez vertelt dat de campagne inmiddels is aangepast. “We hebben een goed gesprek gehad met het NHG en aan de hand daarvan een aantal veranderingen doorgevoerd.”
Aan de nieuwe campagne verandert inhoudelijk niet veel. “We blijven uitgaan van de regel van drie”, zegt Guerrero Paez. “Ons advies: controleer gedurende drie minuten, met drie vingers op drie verschillende plekken.”
Guerrero Paez zegt dat ze begrijpt waar de kritiek van het NHG vandaan kwam. “Het is inderdaad niet bewezen dat zelfonderzoek leidt tot eerdere diagnoses. Maar het ging ons vooral om het leren kennen van je eigen lichaam. Ben je je meer bewust van hoe je borsten voelen, zul je het misschien ook eerder opmerken als er iets niet klopt.”
Waar moet je volgens het KWF op letten bij borstzelfonderzoek?
- Ongewoon knobbeltje in de borst of een bult op de borst
- Verharding van de borst
- Pijnlijke, anders aanvoelende plek in de borst
- Deukjes, kuiltjes of putjes in de huid
- Veranderingen van de tepel met verschijnselen als roodheid, schilfertjes of eczeem
- Ingetrokken tepel (behalve als je die altijd al hebt gehad)
- Verdikt strengetje naar de tepel
- Vocht uit de tepel (bloederig, waterig, melkachtig of groenig van kleur)
- Warm aanvoelende en roodgekleurde borst
- Afwijkende huid van de borst (sinaasappelhuid, schilferig en roodheid)
- Pijn in de borst op een plek waar ook het klierweefsel anders aanvoelt
- Verandering van de huid of bijvoorbeeld een huidzweertje dat niet goed geneest
- Zwelling in de oksel
- Veranderde vorm of grootte van de borst
- Dikke ader in de borst
Self-examination is not a substitute for screening
Mireille Broeders, professor of Personalized Cancer Screening at Radboudumc, understands why people found the original campaign confusing.
“By asking women to check their breasts every month, they can confuse that with the screening we know from the population screening. But that is not the same. Self-examination is not a screening and can certainly not be seen as a replacement.”
Women will soon feel something at a monthly check-up, because breasts can feel different every day, depending on how far along you are in your menstrual cycle.
According to Broeders, screening by means of a breast X-ray – better known as a mammography – can detect breast cancer earlier. The breast is then placed between two plates, after which a digital photo is taken. This screening is mainly intended for women who do not yet have symptoms.
“Breast cancer is visible much earlier on a mammogram,” says Broeders. “While with self-examination you often can’t feel anything until the nodule is closer to the surface of the skin.”
Alertness is always important, not just at one fixed moment
If you know your own body (in this case your breasts) well, you are more likely to feel differences, says Broeders. It is therefore better to see self-examination as something complementary, she says. This also applies to women under the age of fifty who are not yet invited for the population screening.
The professor also thinks it is a good idea not to stick to checking once a month. “If you do, it quickly becomes something systematic again. While alertness is always the motto. I would say: make sure you know your breasts and how they feel, but make the time less important.”
“In addition, many women will soon feel something during a monthly check-up, because breasts can feel different every day, depending on how far along you are in your menstrual cycle,” says Broeders.
The emphasis in the campaign is now on regular checks
The Dutch Breast Cancer Association therefore says that it now emphasizes regular check-ups. “That nuance was important. Our message didn’t come across well because of the way it was formulated,” says Guerrero Paez.
When you examine yourself is personal. “If you don’t menstruate, the moment doesn’t matter. But if you have your period, wait a week until after your period.”
