A respite – and again in battle

Batyr Doskhoev: Three brigades divided the day into three shifts of eight hours. One worked, the other two rested in a clean area. And so 14 days. Then two weeks were on self-isolation, the other three brigades replaced us.

– Did you take the first patient with coronavirus?

– Yes, he entered the infectious ward of the republican hospital, I was on duty. Then he was transferred to the Nazran city hospital, which was made specialized in a new infection. A month of patients was taken there only. The first two weeks the situation in the republic was deplorable. Some doctors got sick, others just stopped going to work. Many doctors and nurses are women; parents and husbands did not allow them to work. Medical staff was not enough. For 60 patients there was one nurse. Patients complained about social networks.

I believe that the covid department should have been initially organized in the republican hospital. She is better equipped, more staff. Then they did so, and the situation began to improve. Patrons, deputies joined. The Ingush people are very friendly, united. If some kind of trouble happens, everyone wants to help. The owners of the cafe brought food, shops – the necessary goods, everyone tried as best he could. Doctors came to work, who initially fell out of rut. The situation has stabilized, and, we must pay tribute to the Ministry of Health, the drug supply in our country throughout the epidemic is one hundred percent.

– Are there enough ventilators? Was there something that you had to pick up from some and connect to others?

– Not once. The largest number of patients on mechanical ventilation was 40. And there are twice as many devices in the republic. In our department (for patients of moderate severity) 12-13 devices were simply standing – there were enough of them in intensive care. In addition to specialized departments in Nazran hospitals, cove hospitals were also deployed in Sunzha and Malgobek.

“Did you get the coronavirus too?”

– Yes, on April 30 I felt that the temperature would rise (37.5), tickle in my throat, and a dry cough started. Since May 1, he was isolated in the apartment and treated. It didn’t come to drugs. I drank vitamins D, C (they give strength to fight the infection and thin the blood) and a zinc preparation (stimulates the immune system). And he drank a lot of mineral water, which contains soda. 30 ml of water per kilogram of body weight is recommended, but I drank almost 100 ml per kg in order to more effectively counteract the infection. When you drink water, viruses from the oral cavity and nasopharynx, where they are concentrated, enter the stomach and die there in the gastric juice.

– Do you think coronavirus can get sick again?

– We didn’t have this, but in Russia a couple of cases were recorded. As we adapt to the new virus, a lot of time will pass. With repeated infection, the disease will most likely proceed in a milder form. Although the virus has not yet been studied enough to be able to say anything.

– What is the trend in the incidence rate in the republic?

– There are fewer severe patients.

“Is the virus getting weaker?”

– No, rather, the doctors adapted. The first two weeks there was a panic state at all. And people arrived on the eighth, tenths, even on the 15th day in serious condition. Now they have become more serious, fewer neglected cases.

“How quickly can deterioration come?”

– I had a patient, 43 years old. Entered with saturation (oxygen level in the blood) 96-97. Norm 95-100. He complained of a dry cough for 5-6 days. The temperature was not high – 37.5. There were no more manifestations. We put it down, took the tests. Literally two or three hours later, a nurse calls and says that he became ill. I come in, and he is all blue. Hands and feet are blue. Saturation is only 78%. They gave oxygen, but its level in the blood did not rise. The patient complained that he was short of air. Transferred to IVL, lay so almost a week. Glory to Allah, climbed out. Managed.

– On the Web they write that you cried, looking at how severely ill patients suffer?

– It was so. In general, I can say, my heart is stony, tears are not typical for me. The last time, before that, cried in 2007. Mom went to Mecca to do the hajj. There she became ill, and she died. Since then – 13 years have passed – not once. But when I looked at the men under two meters tall and weighing 100 kg, I saw how they were tormented and could not help anything … I had tears flowing and I went out of the box so that the sick would not see my face. At such moments, I thought: everything is in the hands of the Almighty. We doctors are just a tool. A healthy person breathes automatically, without thinking about how important oxygen is for him. But, looking at these patients, you realize that you need to appreciate every breath. The dying only begged for this.

“Money is not the main thing”

– You have been chosen as a doctor of the year, how much have you already been working?

– The first of October will be 10 years old. They invited me to other regions, to Moscow, St. Petersburg, Murmansk, Krasnoyarsk, promised a high salary, housing, but I want to help here in my native republic. I do not think that money is the main thing. I have enough for life.

– You somehow were outraged by the prices for medicines in Ingushetia, which were many times higher than in the neighboring republic. “Where are the testers? Owners of these pharmacies are not ashamed? Especially in such a difficult time for everyone. People sit at home, and some even have nothing to eat, ”you wrote on your Instagram. Was there a reaction?

– Yes. Almost immediately I got a call from Rospotrebnadzor, Roszdravnadzor, several prosecutors, including from Magas, Sunzha. Pharmacies were checked, and the situation quickly improved. I’m not afraid to tell the truth.

– It does not bother you in life?

– Anything can happen, including at work. But we must pay tribute to our head physician. This is a woman, but can stand up for herself and for others (supervises the medical institution Lydia Akhilgova – ed.). Since the onset of the epidemic, it has been in operation 24 hours a day. Call me at two in the morning, he’ll come and help. He works with patients in intensive care, trying to provide staff with everything necessary. It is very supportive.
