Since Thursday, April 15, doctors who provide services in large vaccination centers no longer have a choice. Their hours of presence must be paid by the session; or 420 euros for four hours during the week and 460 euros for the same duration on weekends.

The National Health Insurance Fund (CNAM) has put an end to the possibility for them to be paid on a fee-for-service basis, authorized since the start of the vaccination campaign, reserving this formula only for injections carried out in an office or in clinics. health establishments such as nursing homes (welcoming dependent elderly people).

Correct a “windfall effect”

For the CNAM, this new rule is officially part of a concern for “Clarification of the remuneration of physicians”, while the country has passed the milestone of 300,000 vaccines administered per day, 52% provided in vaccination centers. The rise of vaccinodromes, which display flow rates of several thousand daily doses, would justify this concern. “It is not a tool to fight against optimizations”, insists on Medicare.

On the ground, the look is different. And the CNAM’s decision is seen above all as a somewhat belated way of correcting a “Windfall effect”, which has enabled certain doctors to pocket particularly high salaries for their work in vaccination centers.

With a vaccine pre-consultation paid for 25 euros, to which are added 5.40 euros for registration in the national Covid-19 vaccine file, the price is displayed at 30.40 euros for each patient. And at 44.60 euros if the operation takes place at the weekend.

At the rate of fifteen patients per hour in centers organized to be as productive as possible, the remuneration could thus rise to more than 1,800 euros for four hours of actual work – and to more than 2,500 euros on weekends. . With the possibility of linking up to three services in the same day. And to make themselves available for several days in a row.

“A flaw in the system”

The CNAM ensures that it cannot quantify the cost of these practices. But she saw the importance of it enough to put the subject on the agenda for her weekly meeting with organizations representing health professionals on Tuesday, April 13. “There was a flaw in the system”, notes the director of a regional health agency.
