Get to know the difference between white coat hypertension and hidden hypertension

Jakarta: Hypertension is a disease that is quite common in society. If not controlled properly, this disease can be very dangerous. Starting from causing cardiovascular disease, even stroke, so it is not uncommon for this disease to also cause death.

According to dr. Eka Harmeiwaty, SpS, Secretary General of the Indonesian Society of Hypertension (InaSH), to reduce morbidity and mortality due to hypertension, early detection is needed in the adult age group aged 18 years and over.

“In the field sometimes there are problems in establishing a definite diagnosis of hypertension because from the measurement results there are other categories, namely white coat hypertension and masked hypertension,” said dr. Eka in the Virtual Press Conference: Management of Hypertension at Pandemic Period.

White coat hypertension, said dr. Eka, often found in patients with grade 1 hypertension (siatolic blood pressure 140-159 and / or systolic pressure 90-99 mmHg) on examination at the clinic. However on home measurements the blood pressure was normal.

“This individual does not need to be given treatment, but long-term monitoring is needed because of the risk of developing hypertension in the future. The prevalence is estimated at 2.2 – 50 percent and is strongly influenced by the method of measurement in the clinic,” continued dr. Eka.

On the contrary, according to dr. Eka, hidden hypertension showed normal blood pressure when checked at the clinic. However, measurements outside the clinic showed elevated blood pressure. From various studies the prevalence is 9-48 percent.

“Covert hypertension has a high risk of organ damage. To know white coat hypertension and hidden hypertension, it is necessary to check blood pressure at home, hereinafter abbreviated as PTDR,” explained dr. Eka.

“In the midst of the PTDR pandemic, this is very useful because some patients are reluctant to go to the hospital, especially elderly patients. The results of the PTDR can be consulted with the treating doctor online either by chatting via social media or telemedicine, “said dr. Eka.

According to him, this PTDR is recommended for hypertensive patients, especially for hypertensive patients with kidney disorders, diabetes, and pregnant women as well as patients with poor medication adherence.
