COVID-19 is a dangerous and insidious infectious disease caused by a virus that a person has never encountered before. There is still no vaccine from it, and there is no clear list of drugs that will certainly cure or prevent infection. Moreover, nature will not allow the virus to kill all of humanity, because then it will have nowhere to live and multiply. The only ways to protect yourself and loved ones from an unpredictable disease are to observe hand hygiene, maintain social distance and follow restrictive measures. The only way to avoid the explosive nature of the development of the disease. The chief freelance epidemiologist of the Ministry of Health of the Chelyabinsk Region, candidate of medical sciences Alexander Vygonyailov, is sure of this.
– Alexander Vitalievich, what is a coronavirus pandemic? How is it fundamentally different from the pandemic of the same highly pathogenic flu that was in 2009?
– Both that, and another are really pandemics when the virus covers all continents. But there are differences, and they are significant. The 2009 pandemic occurred as a result of the reassortment (mixing – approx. Ed.) Of human, poultry and swine influenza viruses, resulting in the emergence of a new АH1N virus. And he, too, turned out to be highly contagious, and quickly spreading. But humanity has had the experience of producing a flu vaccine, but it is still a flu virus. And the vaccine in that case was developed in a short time – a pandemic was announced on April 25, and by the fall of 2009 the vaccine had already appeared.
There is no vaccine against coronavirus. And never until today the question was not raised, although coronaviruses have long been known, but until 2002 it was believed that they could not cause any serious diseases.
They circulate, three strains of coronavirus are among the 200 pathogens of acute respiratory diseases. ARI is not one diagnosis, but, in fact, two hundred different, therefore, the symptoms will be different for everyone.
Yes, in 2002, coronavirus showed teeth when one of its strains caused an outbreak of SARS, which is also called SARS (from the English Severe acute respiratory syndrome – Severe Acute Respiratory Syndrome (SARS) – approx. Ed.) In southeast countries Of Asia. And here is the same story of origin as it is today – bats, China. Then in 2012 there was MERS (Middle East respiratory syndrome – Middle East respiratory syndrome – approx.ed.), Which also caused coronavirus. By the way, it is the countries of Asia that often give us such infections.
– And due to what, in your opinion? Is it crowding or lifestyle?
– Crowding of the population, warm and humid climate, cultural characteristics, including the presence of the same bats in the human diet. Well, and, as in the whole world, a completely ugly human invasion of the familiar habitat of animals. These facts today contribute to the fact that we meet with those viruses that previously circulated only among animals. A person began to constantly contact them as a result of an invasion of the wild, then the viruses crossed the species barrier – and this is a very variable microorganism that appeared earlier than us and will probably live longer – and adapted to life in the human body. In general, the more we invade the wild, the more we will have such consequences.
– That is, we naturally received an answer: do not meddle where they do not ask.
– In general, yes, or, in any case, be more delicate. I note that the same bats have six more coronaviruses with the same potential as the existing one. In general, we do not know 99% of the microworld, we only know that it exists. And all of it can adapt.
– What is the danger of coronavirus in the global sense?
– The peculiarity of this coronavirus and the difference between its highly pathogenic flu is the unpredictability of its behavior in the body of a particular person. On the one hand, many people suffer the disease very easily or without any symptoms. But such people are the epidemiological core of the spread of the disease. 80% of the infection is easy, 20% of them without symptoms.
On the other hand, the virus very quickly infects the lung tissue due to the fact that it attaches specifically to the receptors that are in the alveoli.
We can recall the Diamond Princess ferry, which was quarantined in the port of Yokohama (Japan), together with 4 thousand passengers, because of a coronavirus-infected passenger. The Japanese set up a very cruel experiment, but it very clearly defined the features of the epidemiology of the virus: how many people can get sick if no restrictive measures are taken. There, 20% – 712 people fell ill. But even some of those who were ill easily had pneumonia damage to their lung tissue.
– Why in 2002, when after the SARS epidemic it became clear that coronaviruses are dangerous and cause more than acute respiratory infections, were no measures taken to develop a vaccine for coronavirus?
– You need to understand that these are different viruses of the same family. That coronavirus quickly disappeared, although by the number of deaths, more than 20%, and the severity of the course of the disease, it was more aggressive. We do not see such mortality even in today’s Italy, where it is about 12%, and then, I think, this is an overestimated figure due to the underestimation of mild and low-symptom forms. But there were only 30 countries. One way or another, the vaccine was not developed. COVID-19 acquired a pandemic character and quickly spread throughout the planet. China announced the epidemic on December 31, WHO recognized the problem on January 25, and a pandemic was announced on March 11.
The influenza virus, even highly pathogenic, does not affect the respiratory system in such a volume. Here the problem is that, due to clinical manifestations, very large medical forces are needed to fight the new coronavirus: these are hospital beds and oxygen support, which has never been needed in such a volume. In addition, this is a long treatment in a hospital, including in intensive care units. One person blocks a bed for at least two weeks.
And, of course, the lethality of this infection is much higher than the lethality from influenza.
In addition, COVID19 causes a large number of pneumonia, both viral (under the action of the virus itself) and secondary bacterial.
– They say that the vaccine will appear no earlier than the end of the year …
– The new coronavirus is a completely independent virus with which humanity has never encountered, therefore, at the first stages there were no protocols, no experience, no developments, and even less a vaccine. There are already vaccine prototypes: six in Russia and in the world as a whole – about 20. And the vaccine is not so difficult to produce, it is important to prove that it is effective and safe. Tests are carried out first on animals, then clinical on volunteers. We need to see how long the immunity lasts, how long it takes to build up. According to the latest data, the prospect of releasing the vaccine is already in circulation – indeed the first quarter of 2021, especially since Russian scientists are ready to begin clinical trials.
– You said that 80% tolerate the disease easily. Who, according to today’s research, can be attributed to 20% of the risk group?
– This is a very selective virus. Basically, it affects people, and not even so much the elderly, but people with diseases of the cardiovascular system, endocrine system. Of course, people over 65 are more likely to have this pathology.
And the next moment.
In epidemiology, there is such a rule: the more any pathogen circulates, the greater the likelihood of growth of its aggressiveness, since it needs to survive.
Therefore, of course, this is due to the fact that young people became sick more often, but the state of health of the young generation contributes to this. But this is still a hypothesis; it takes time and research to confirm it. Although in general, people who have been ill and vaccinated in the near future will create collective immunity, and the virus will weaken. We see this with the example of a pathogenic influenza. I add that now there is evidence that mortality among men is higher. A parallel is drawn with the fact that among men there are more smokers, and smoking weakens the pulmonary system.
– But they said that the virus does not take smokers …
– It was a false theory. These are all haste and statistics errors. And when the study is not planned correctly, such a statistical error appears.
Most studies show that smokers get sick harder. At the same time, in men, if we talk about mortality, the level of cardiovascular disease is higher than in women. Women of childbearing age can be said to be protected by nature.
– Are children also less likely to get sick?
– The susceptibility to the virus is general, regardless of age. Children get sick no less, they get sick easier. It is a fact. In our Chelyabinsk region a seven-month-old child was ill, but it is almost asymptomatic. But again, we must understand that if a sick child has any concomitant diseases, then an unfavorable outcome is possible.
– This is also, rather, from the category of rumors so far: does BCG vaccination really reduce the risk of a severe course of the disease?
– This is not a rumor. American scientists conducted a study that showed that individuals vaccinated against BCG have cellular immunity. Their immune response is more harsh and adequate to the virus. Only this can be explained. But you can not say: if you are vaccinated against BCG, then you will not get sick. And all the more stupid, upon learning this, run and get vaccinated. This is not necessary. BCG is a live attenuated tuberculosis vaccine, and the main indications for its use are vaccination of children in order to prevent generalized forms of tuberculosis in newborns that are fatal to them. This is just one study, and no matter how it turned out like with smokers. So far there are no such multicenter studies, but they do not need to be accelerated either, since this problem will not solve here and now and does not give us an effective means of prevention.
– But in principle do you need to get vaccinated?
– Of course.
You need to be vaccinated constantly. WHO has recognized that there have been two effective measures over the life of healthcare — vaccination and clean water.
With the last in the world, and in Russia, it is very difficult, but with vaccinations we can still protect ourselves. And in the twentieth century, life expectancy increased by 30-40 years only due to the wide spread of vaccination. But there are vaccines only against 35 infections, and there are more than 2000, in addition, we constantly see new ones.
– You say that the world has received the coronavirus pandemic not only due to imported cases, but also due to infection already within the country and within the region. In this case, will Russia not get the Italian script now, or what is happening in the USA?
– We won’t get the Italian scenario and the US scenario for sure. The fact is that we introduced those restrictive measures that they did not take for a long time rather quickly.
Another conversation is that now, in my opinion, the need has arisen for Russia to introduce tough interregional restrictive measures, as China did.
While we do not see this fully. And, probably, we should think about this, since the focus in Russia now is Moscow and the Moscow region, where the epidemic is already developing along an uncontrolled path. The same St. Petersburg is far from Moscow in terms of disease.
– If now we completely close the Chelyabinsk region for entry and exit, we will not get explosive growth of the sick?
– Maybe it’s not even quite close the region, but to make the movement of the population fully controlled. And the measures that we see at the regional level, they are correct, including recommendations to those who come from other regions, especially from the capital regions, to report about themselves and get under medical supervision.
But at the same time, even in our 50 cases, almost two-thirds fell ill within the region. Moreover, in seven cases we do not clearly know the source of infection – these people did not go anywhere and were not in contact with the identified cases.
The remaining cases are close family contact, which is generally characteristic of the virus. 75% of infections worldwide occur in family foci.
– For example, the press secretary of the governor Sergey Zyu, who spent 80% of his time at work.
– Yes, he is one of seven.
“But as it is, his whole environment has negative test results, and his disease is confirmed.”
– Perhaps, somewhere, a person, not specifically a spokesman for the head of the region, does not always evaluate all the risk factors for infection. Therefore, I repeat once again, restrictive measures are the most reliable way to prevent the spread of the disease at this time. If you do not cite Italy and America as an example, then after 80 days of restrictive measures we can reduce the number of infected by 6 times. And the strict restrictive measures in the foci of infection that China carried out in Wuhan showed almost impossible: the number of possible infections was reduced almost 100 times in the same 80 days (in Wuhan 79 days were tough measures – approx.ed.). So, there is COVID, it is distributed, in the absence of restrictive measures, one patient infects, according to statistics, two or three people, these people are another two or three, and so on. Every seven days, the number of infected people doubles. If we introduce restrictive measures, then one patient already infects one or two people and, in general, in 80 days we slow down the development significantly.
Moreover, 20% of those infected, however much we would like to, will be seriously ill and will need mandatory hospitalization and high-tech care.
– Closing borders and controlling the flow is one thing. But what to do next if infection within the region is already underway? As they say to us: to sit at home, not to walk on the playgrounds? Is the regime of universal self-isolation really necessary?
– The most important, fundamental in our case, is distance. Of course, walks in the fresh air should be, they are necessary, but it is important to keep a distance: the distance between people in all cases should be 1-1.5 meters.
– What is still the most likely way to transmit the infection?
– The transmission method is airborne. The main one is drip. This is the difference between coronavirus even from the same tuberculosis, where the main method is aspiration. A sick COVID-19 person secretes a virus when talking, sneezing, or coughing. It is not in the air, because this virus, roughly speaking, is a large drop and does not fly far. Hence the distance of 1-2 meters, which is safe. To reduce this distance, the sick person should always use a mask. The virus falls to the surface and lives on it. This, by the way, is the difference from the flu virus, which dies quickly enough on an open surface, within 12 hours.
A new coronavirus can live up to five days on paper, on plastic. Just at this moment, the second transmission path is activated – the contact one, when we do it with our own hands we put it into our mouth, nose or even our eyes.
This was clearly shown by the story of a doctor from China, who was the first to talk about the problem. He worked without glasses. An aspiration route is also possible, but this is more with medical care, when a fine aerosol is formed during the manipulation. These drops easily penetrate the mask and can be dangerous at a distance of up to 5 meters.
– For how long does a person with a coronavirus remain contagious?
– A person becomes infected two days before the end of the incubation period, which lasts from two days to two weeks. On average, it is five to seven days. The first three days of the disease itself are highly contagious. In severe cases, a person is contagious for much longer than two weeks.
– Is it possible to get sick a second time and are antibodies to coronavirus produced?
– We can talk about the development of antibodies only theoretically, test systems have been recently registered in the Vector laboratory. They plan to start the examination with medical workers in order to understand and minimize the risk of their professional infection. But, again, if Italy simply brought the situation from such recklessness to such a trend, then in the USA they hoped just that the population would easily get sick and develop collective immunity, then there would be no need to invent anything.
Collective immunity must be developed by vaccination, but so far there is no vaccine.
The fact that you can get sick a second time – there is no such data.
– How can we protect ourselves from infection?
– Firstly, to move away from a person, no matter whether he is sick or not, by one and a half meters. Even if he is sick, you will not get infected. The next point is hand hygiene. To do this, it is enough to have an alcohol-containing antiseptic with you, and it should have at least 70% alcohol.
– That is, vodka will not work?
– No, only 40 degrees in vodka, but if there is no alcohol, it is better to wash your hands. But they also need to be washed correctly: necessarily interdigital space, fingertips, thumbs, that is, those places where dirt accumulates most. Hands should be handled even if you wear a mask. Вы должны перед ее снятием, именно снятием, а не сдвиганием на шею или на лоб, помыть руки, перед одеванием — снова помыть руки, потому что вы могли их контаминировать (загрязнить — прим. ред.).
— Нужна ли вообще маска в таком случае?
— Маска нужна только в закрытых помещениях, но при этом нужно помнить: маска защищает два часа и должна сниматься только с последующей обработкой рук. Если вы коснулись маски руками, то ее нужно снять, а руки обработать спиртосодержащим антисептиком или тщательно помыть с теплой водой и мылом. В остальных случаях нужно просто соблюдать гигиену рук.
— А если ходить в перчатках?
— Надели мы перчатки и думаем, что они нас защищают. Но ведь перчатки тоже контаминированы. В общественном транспорте сидит человек в перчатках, хватается ими за поручни, сиденья, а потом то нос почешет, то глаз, то в носу поковыряется. Непроизвольно. А это способ передачи. И маска то же самое — на ней собирается вся грязь. Вирус, как мы помним, живет на бумаге, ткани, на латексе — чуть меньше — до трех суток. Поэтому проще просто обрабатывать руки.
— Сейчас появился тренд именно на постоянное ношение масок, в том числе тканевых, дизайнерских. Защищают ли они?
— Маска, повторюсь, нужна только в закрытых помещениях, где вы не можете соблюдать социальную дистанцию. Носить ее постоянно не только не нужно, это не гигиенично. Вопрос о том, защищает ли тканевая или марлевая маска, дискуссионный. Мы помним 90-е годы, когда в больнице медперсонал носил четырехслойные марлевые маски. Постирал их, поутюжил и дальше носишь, пока не развалилась. Сейчас практика четко показывает — не защищает такая маска от коронавируса. Для защиты нужно делать ее в 8-10 слоев, но так дышать будет невозможно. Кроме того, от стирок качество теряется. И ВОЗ четко сказал — марлевые маски не рекомендует.
Поэтому все эти кустарно сшитые маски, если мы не проверили их уровень защиты, вызывают как минимум вопросы. Безусловно, если вы находитесь в помещении и рядом с вами больной, это лучше, чем ничего, но не надо на нее уповать.
Основное — дистанция не меньше метра и обработка рук, в том числе после контакта с наличными деньгами.
— Многие переходят к крайностям: после похода на улицу или в магазин проводят дезинфекцию дома, оставляют на улице одежду и обувь. Оправданы ли эти меры?
— Вирус способен выживать на поверхности, но обладает низкой устойчивостью к средствам дезинфекции. Можно просто проводить влажную уборку дома с моющими средствами, потом хорошо промыть тряпку и обязательно ее просушить. И, конечно, проветривание — хотя бы каждые два часа по 10-15 минут. Это тоже надежный способ защиты от вируса. И, главное, если ты чувствуешь недомогание, вызови врача на дом, изолируйся от домашних и не выходи из дома.
— То есть самолечением даже при легких симптомах простуды не стоит заниматься?
— Безусловно. У нас уже четко прописано в стандартах: подозрением на COVID-19 считать любое повышение температуры выше 37,5 градуса и наличие респираторных симптомов, так как сейчас не сезон ОРВИ и гриппа. Вообще, у нас сложилось очень легкомысленное отношение к ОРВИ и ОРЗ, хотя из всех инфекций, будь то ВИЧ или туберкулез, наибольший экономический ущерб наносит как раз ОРЗ из-за своей массовости. 90% в структуре заболеваемости инфекционных заболеваний — это ОРВИ и грипп. Поэтому ни в коем случае нельзя заниматься самолечением, а сейчас — тем более. Обратившись к врачу, вы защитите не только себя, но и свое окружение.
Нужно помнить, что коронавирус очень коварен. Он может начаться легко, но за короткое время перейти в тяжелое течение.
И те пневмонии, которые мы видим по СOVID, все они начались с респираторных симптомов, которые ошибочно трактовали как легкую простуду.
— Москва говорит, что по пневмониям очень много ложноотрицательных результатов на коронавирус, то есть при имеющихся клинических симптомах, тестирование дает отрицательный результат.
— Ни при одной инфекции ни один тест никогда не имеет 100-процентную точность. Зависит от правильности забора биоматериала, своевременности доставки, даже материала, в который поместили пробу. Все это может сказаться на ложноотрицательных результатах.
В целом считается, что мазок из носа дает только 36% от истинного количества положительных тестов, из зева — 63%. Поэтому их берут вместе и складывают в одну пробирку. Мокрота — 72%. Если при этом нарушена процедура забора, время доставки, то мы этот процент еще снижаем.
Этот вирус еще и плох тем, что у него нет явно специфических симптомов. В целом — это ОРЗ и пневмония. И даже результаты компьютерной томографии не говорят конкретно о повреждениях легких коронавирусом нового типа, они говорят о том, что это вирусная пневмония, определить именно пневмонию, вызванную коронавирусом, можно лишь в ряде случаев. Тем не менее по новому приказу минздрава мы всех пациентов с пневмониями маршрутизируем только через приемно-сортировочные отделения, где им проводится КТ. И есть такие случаи, когда нет ни эпиданамнеза, ни сопутствующих факторов, но есть тяжелая пневмония с обширным поражением легких, тогда мы тоже поставим диагноз «коронавирусная инфекция».
При этом, конечно, не хотелось бы переборщить с массовой постановкой таких диагнозов, потому что это серьезная и затратная ситуация: и отделение, где лежал такой пациент, нужно закрывать на обработку, и коек у нас не безграничное количество, даже для пневмонии.
Да, сейчас мощности наращиваются. Для коронавируса только в Челябинской области 2600 коек. Там, конечно, есть сложности. Это ведь не только аппаратура, это разводка кислорода, средства защиты для персонала, обучение медиков с целью привлечения непрофильных специалистов к лечению инфекционных больных.
— Когда мы ждем пика болезни и спада?
— Это, можно сказать, зависит от каждого из нас. Ограничительные меры при их исполнении в полном объеме должны позволить выйти на пик к концу апреля. Если мы выйдем на пик к этому времени, то установится плато, то есть какое-то время количество заболевших за сутки будет примерно равнозначным, без резкого роста, а затем пойдет падение. При таком прогнозе в мае это будет плато, а во второй половине июня спад.
— А в Челябинской области?
— У нас пока сохраняется контролируемая эпидемия, о сроках говорить сложно. Но риски заноса из Москвы очень высоки. Плюс ко всему, мы поздно закрыли границы с Европой. Мы надеемся, что те ограничительные меры, которые предпринимаются, и социальное дистанцирование дадут результат. Другого пути у нас нет. Если отпустить контроль, коек на всех не хватит, сил медработников тоже.
Каждый человек должен понять одно: заболеешь или нет — зависит только от тебя. Конечно, медицина сделает все возможное, чтобы вылечить, но возможности не безграничны.
Взять те же 2600 коек, которые создают у нас, перепрофилируя медучреждения, это колоссальные объемы. ОКБ-2 перестала оказывать помощь тем, кто в ней наблюдался. Целый район. Отодвинуты плановые операции, и надо понимать, что не для всех отстроченная на время помощь придет вовремя. Человеку просто нужно понять и принять это: дистанцию, самоизоляцию. По сути, человеку впервые дана возможность спасти мир, лежа на диване. Не видно возбудителя, поэтому кажется, что он далеко и что 50 человек на 3,5 млн жителей Челябинской области — это крохи. Но все очень быстро может измениться, если мы не будем следовать тем мерам, которые определены.
— Как нам повысить иммунитет, если гулять толком нельзя?
— Дышать воздухом можно, но не гуляя парами и не на шашлыках на природе в компании. Физическая активность должна быть, но разумная. Никто не отменял зарядку дома, проветривание, чтобы подышать свежим воздухом. Хотя найти такой воздух в Челябинске — это задача очень сложная, и непонятно, что лучше: сидеть дома или дышать этим воздухом. Особенно мне «понравилось», как администрация города проводит дезинфекцию улиц при помощи аэродромной машины. Вообще-то, Роспотребнадзор четко прописал, что дезинфекция открытых пространств аэрозольным методом запрещена. Можно было просто распылить дезинфицирующее средство теми же поливочными машинами, только прежде промыть дороги чистой водой, а не поливать грязь и пыль.
— Да, может быть, стоит хорошо и регулярно мыть улицы обычной водой?
— Перекись, безусловно, эффективна. Но для начала дороги нужно помыть, потому что грязь дезинфицировать бесполезно, любые дезсредства теряют в этом случае свою активность. Кроме того, при том уровне распространения заболевания в Челябинской области, это еще и не столь необходимо. Вообще, если и обрабатывать, то это нужно делать регулярно и два раза в день. В другом случае это совершенно ненужная активность.
А вот что делать уже нужно — так это дезинфекцию подъездов и, в первую очередь, дверных ручек и лифтов, особенно кнопки. Их почему-то никто не моет с дезсредствами, и вот как раз здесь бы я выставил управляющим компаниям не рекомендации, а требования, поскольку мы все хватаемся за эти ручки подъездов в отличие от асфальта, по которому мы не водим руками, а обувь можем помыть дома.
— Нужно ли здоровым людям для поднятия сопротивляемости иммунитета пить иммуностимуляторы или противовирусные средства?
— Бесконтрольно употреблять такие препараты нельзя. У всех у них действие примерно одно: стимуляция всех звеньев иммунной системы, что напоминает ситуацию, когда пытаются гнать загнанную лошадь. Если иммунная система дает сбой, то эти препараты ее просто подстегнут, но не поправят и последствия могут быть непредсказуемыми. Нужно просто вести здоровый образ жизни: сохранять режим сна и отдыха, время приема пищи, лечить или контролировать имеющиеся заболевания. Сейчас можно рекомендовать при приходе с улицы мыть руки, умыть лицо и промыть нос. Это банальная механическая дезинфекция.
— Тоннами есть имбирь не нужно?
— Нет. Фактов, подтверждающих, что если ты его съел, то будешь защищен от нового вируса, нет. Если он тебе нравится — ешь, какие-то антисептические свойства у него, конечно, есть, так же, как и у лука с чесноком. Последние еще и позволят наверняка соблюдать социальную дистанцию — к вам никто не решится подойти.
Есть данные, что рекомбинатные интерфероны могут защитить от коронавируса. Но с ними также надо быть очень осторожными: начинаешь ты капать чужеродный интерферон, а твой прекратит вырабатываться. Если это и делать, то под контролем и короткими курсами. Других средств, в том числе медикаментозных, пока не разработано. И еще раз напомню, что любые препараты должен назначить врач, оценив показания и возможные риски.
— ВОЗ приводит статистику по смертности от других заболеваний, тот же ВИЧ или туберкулез, или иные болезни, и на фоне этого коронавирус выглядит, скажем так, не очень грозно. Так ли страшен черт, как его малюют?
— Мы пока не можем оценить смертность от коронавируса, потому что пандемия не окончена, часть людей болеют и не все из них выздоровеют. Да, смертность у нас в России меньше 1%, но можем ли мы ее сравнивать с Италией, ведь у нас и эпидемия началась позднее. Показатель смертности как в Италии или США — это 12% — такого нет ни у одной другой инфекции. Об этом надо не забывать. К тому же коронавирус может косвенно стимулировать смертность от других заболеваний. У онкологических больных — это проблема, для больных сердечно-сосудистыми заболеваниями — проблема, для больных сахарным диабетом — тоже проблема. И от этих заболеваний смертность вырастет, если мы не будем ничего контролировать. Сказать так, что сейчас ничего не надо делать по коронавирусу, так как есть другие болезни, — это неправильно. Все взаимосвязано.
Совершенно правильно, что мы должны предотвратить взрывное распространение болезни.
