Coronavirus. At the Rouen University Hospital, a situation “under control” according to Professor Benoît Veber

We learned of the death of several doctors during the last 48 hours. How is this news received in the medical community ?

Benoît veber : “It’s traumatic for caregivers, of course. We are affected, but I would also say that we are very focused. This reminds us that everyone can be affected and that we must remain vigilant about all protective measures. We hope that the awareness, which is very strong today, will help limit these dramas. “

Where is the situation at the Rouen CHU, resuscitation side ?

“The situation remains completely under control. According to the latest data that I had in my possession on Monday noon, we only had two Covid-19 patients in intensive care at the CHU in Rouen. So we are far from overwhelmed. With us, there are four adult resuscitation services: medical and surgical resuscitation and two specialized resuscitations, neurosurgical and cardiac surgery. Medical resuscitation is responsible for welcoming patients in distress during the initial phase: meanwhile, surgical resuscitation will welcome non-Covid patients. And if medical resuscitation is never enough – which is possible when we look at what is happening in eastern France -, at that time, surgical resuscitation will also be dedicated to Covid-19. And the two specialized resuscitations would in turn become a more versatile service. In addition, there are expansion possibilities with, for the moment, two modules of ten intensive care beds. In total, in summary, there is a potential for around 70 Covid resuscitation beds, which you can count on right away. “

When do you expect this famous dreaded wave ?

“The Covid-19 units in medicine are filling up, so the virus is circulating. And as we know that the worsening is between the fifth and the eighth day, it is possible that the cases will flow in intensive care within a few days. This wave is currently affecting Paris. Like us here in Normandy, the Parisians were able to take advantage of their few days of observation before it arrived, which means that we do not hear less about great difficulties in resuscitation, even if it saturates slowly. You should also know, in the case of the Covid-19, that it is a question of long resuscitations: three weeks at the very least, when really it is not going well. So the beds are blocked for a long time. That’s the problem. “

Will the Rouen University Hospital receive patients from the most affected regions ?

“We were asked this weekend to welcome between four and six resuscitation patients from eastern France. We have agreed in principle, but I think this is under discussion as to whether or not they really need to be discharged. “

The lack of equipment has been repeatedly emphasized in recent days. Including, in some places, a lack of specialized equipment for resuscitation. What about Rouen ?

“In Normandy and Rouen, in intensive care, this is not a problem at all. In some hospitals in northern Italy and possibly eastern France, what may have been missing are artificial ventilation. Currently, here, we are far from this situation. We have respirators in all our resuscitation rooms: we even have more, since as part of the “white plan”, we have sanctuarized ten heavy respirators. Afterwards, in terms of hydroalcoholic solutions, this is something that is not very complicated to manufacture: the pharmacy of the CHU should be able to quickly produce 5000 liters, which represents about twenty days of consumption. As for the masks, we consume about 20,000 per week … According to my information, 100,000 masks have recently been delivered to hospitals throughout the whole of Upper Normandy: therefore, we have a little advance, but it is true that it is an important commodity, and the situation would become very problematic if they ran out. But I cannot believe that France will not be able to manufacture millions of masks in the coming weeks. So I’m not particularly worried about this subject. “

What inspires you from the debate on the potential benefits of chloroquine ?

“We’re following this very closely, of course. In fact, Covid-19 is a general lung inflammation that can take the sick away. Chloroquine has an anti-inflammatory effect. But I do say “would”. A scientifically well conducted study is underway. The one done in Marseille, by our colleague, is a pilot study which has the merit of launching an alert, but which does not allow to affirm that it is 100% effective. This is not what we call, in medical terms, a controlled study. Do not get carried away, because this drug has side effects: they should not exceed the expected benefit of the treatment. I understand the popular enthusiasm for finding an effective treatment, but beware: there have already been significant heart problems associated with taking this drug, especially when taken wildly in people who panic. “

Did France show carelessness, in your opinion, in the face of increasing risk ?

“Perhaps at the start of the crisis, when the virus hit China, Europeans observed it from afar. But it’s been a while since the doctors became aware of the problem. Especially since his arrival in Italy: we have had worrying alerts, not necessarily disseminated by the general press. I’d say it’s been two months since we realized that something very serious was going on. “

Medical students “eager to help”

Podcasts,
power point sound systems, digital classes … Although the medical school has closed, ”
educational continuity has been implemented ”, digitally, ”
on the six years of medicine “, recalls Benoît Veber, this time in his cap as Dean of the Rouen faculty. At the same time, the students were put to work in the field (on a voluntary basis) to deal with the crisis.

Our students are very positive, very eager to help, says the professor.
They do not hide behind their books! “ Concretely, ”
two promotions are currently being solicited ” in third and fifth years, specifies Benoît Veber. These students are currently – or will be – ”
redeployed to help regulate in the three Samu of eastern Normandy – Le Havre, Rouen and Évreux -, to give advice on clearly identified protocols … Some are also redirected to the reception of emergency hospitals. They are also sent to health centers, where they help general practitioners with switchboard, reception, hygiene advice, etc. Others, finally, are directed towards the Pass [Permanence d’accès aux soins de santé, NDLR]
, for helping the homeless “.

There are also sixth year students, ”
in reserve because they have internship in three months “, says Benoît Veber. The latter will however be solicited
i the situation becomes catastrophic “.

In any case, from an academic perspective, this coronavirus crisis should be instructive. ”
There will certainly be positive things to learn, yes, unfortunately beyond human dramas. Especially in terms of the renewal of teaching methods. We’ve been talking about digital education for a long time : modern tools are being developed today which may be more suited to student demand. “

According to the testimony of the dean of medical school, teachers saw more questions and participation from students behind their computers, rather than in the intimidating setting of the amphitheater. ”
But we cannot do that, nevertheless warns Benoît Veber.
There is still a need for contact! “

“Your life, our fight! “

After a weekend of rest, Karim Mameri got into gear on Monday “ for a long week ” “ Our tour [celui de la Normandie, NDLR] will happen “, Assures the president of the Council of the order of the nurses of Eure and Seine-Maritime.

It is starting to get seriously serious in the region “, Adds the nurse, who also works at the intercommunal hospital center in
Elbeuf-Louviers, where four Covid-19 deaths have been confirmed to date. On behalf of ”
Reserve duty “,
Karim Mameri will not say more about the situation within the establishment.

More generally, ”
this is not the time for controversy “, He insists. Even if it is not a question of silencing the difficulties encountered on the ground, or ”
anger Sometimes felt by his colleagues. Especially when ”
we’ve been told for weeks that the masks are on the way and we still don’t see them coming

This lost time, we will pay it

But according to the nurses’ representative, the situation has improved over the past three days. ”
368 000 masks – surgical and FFP2 – have been delivered or are in the process of delivery with logistical support from the Sdis [Services départementaux d’incendie et de secours, Ndlr]
Norman, for liberal professionals. We also need them to keep the hospital running.

In hospitals, needs are still being felt, in particular ”
in protective glasses, in alcoholic solution, in sterile gownss “, list Karim Mameri, but”
I tend to be optimistic “, He reassures.

And despite the legitimate apprehensions, the nurses are there, says their regional spokesperson. ”
Your life, our fight ! “: This is the motto of the army health service, taken up by the president of the council of the interdepartmental order, for the attention of his colleagues. ”
We do the best job in the world, we are all proud of it. And it was not his 1.96 m, per 100 kg, that prevented Karim Mameri from shivering to the sound of applause, on the way to his home on Friday evening.

Paradoxically, he says,
I have heard from some colleagues who face the distrust of other residents, in their buildings. “While breaches of containment have been widely commented on and criticized in recent days,”
I feel like everyone today is really aware of the issues “, Comments the president of the council of the order of the nurses of Eure and Seine-Maritime. ”
The concern is that these errors and the time lost in containment, we will necessarily pay.

On-call number

The Eure and Seine-Maritime nursing association commissioned a standby telephone line for professionals : 07 80 97 55 01.
