The doctor, also known to fans as McDreamy, appeared in a dream of Meredith Gray, his great love in the series played by Ellen Pompeo. In an interview with Deadline The actors and showrunner Krista Vernoff have announced that they wanted to make the corona special, which was dedicated to healthcare workers, extra special with the reunion. They also wanted to give viewers a moment of pleasure in a broadcast that was otherwise quite depressed.

In the season opening, the hospital in which the series is set was filled with corona patients, which put a heavy strain on the staff at Gray Sloan Memorial. Vernoff previously announced that it was quite a challenge to film the series now that there are many corona measures on set. Often scenes with different actors are filmed separately as a result, but Dempsey and Pompeo confirm that they did record the dream scene, which takes place on a beach, together.
