Doubs: a new territorial professional health community

To allow better organization of care in the territories according to the needs of their population, to improve the conditions of exercise of health professionals, to better structure the care paths of patients and to contribute to the improvement of their care: these are the missions of the professional territorial health communities (CPTS).

From 2019, in the continuity of the group work carried out within the local hospital of Ornans within the framework of the local health contract, the health professionals of the Loue Lison territory have initiated a reflection on the creation of a CPTS. The objectives of this community are multiple : better knowledge, the pooling of skills, a common reflection on the improvement of patient care, preventive actions for the inhabitants of the territory.

The health crisis of the past two years has demonstrated the importance of better organization and collective action by professionals organized to deploy vaccination, contribute to screenings, etc.

On the strength of this experience, the CPTS has oriented its health project around three axes:

  • Improve access to care by facilitating the use of a general practitioner

It is a question of allowing all patients aged 16 and over who do not have a treating doctor to access care, in particular patients in fragile situations (long-term illness, elderly patients, patients in precarious situations ). The objective is also to strengthen the management of unscheduled care.

  • Organize the multi-professional career around the patient

This mission targets in particular the course of the elderly patient, to encourage them to stay at home and avoid interruptions in care, and dedicated courses for diabetic patients.

  • Develop prevention

Two themes are already priorities : promote cancer screening and vaccination.

The CPTS brings together around fifty members : private health professionals (general practitioners, pharmacists, dental surgeons, nurses, physiotherapists, speech therapists, psychomotrician, dietician, midwife, podiatrist-podiatrist, psychologist, psychomotrician), establishments and services (local hospital of Ornans , Quingey hospital, coordination support system).

The community also works in collaboration with the medico-social structures of the territory or intervening in the territory (ESAT, EHPAD, IME, etc.) and hospitals (CMP de Novillars, adolescent house, mental health platform, HAD, etc.), who participate in this dynamic.
