Once this is done, the person responsible for making the card will fill out the application with the data that you have previously delivered to your usual place of residence. After entering your social security number, the heads of the health center will ask you for your provisional address and will ask you how long you have estimated to stay there. When you have filled in all this information, you will receive a document proving that you have the displacement health card. With this card, and with the serial number that you will find on it, you can make an appointment to get the COVID-19 vaccine in the next few days.
