Birth Certificate Service

Wake County Register of Deeds and Wake County Regional Centers offer birth certificates that occurred in Wake County. Please call 919-856-5460 or email [email protected] for more information. For births that occurred in other NC counties, contact the Register of Deeds for that county or the office of NC Vital Records al 919-733-3000.

To obtain a birth certificate, one of the following requirements must be met (A, B, or C)

  1. The applicant must be an immediate relative of the individual on the birth certificate.
    Applicant can apply for himself, his wife/son, sister/no, children, parents, step-parents, grandparents or grandchildren.
  2. If the applicant is seeking legal information to determine rights of person or property.
  3. If the applicant is an authorized agent, attorney or legal representative of one of the persons in section A or B.

Choose one of the following options to obtain a certificate of birth that occurred in the Wake County:

To request a birth certificate online

  • Make sure you meet the above requirements.
  • Complete the form online Wake County Certificate Request.
  • Use: The cost is $10 for each certificate, plus an additional charge by the service provider company.

To request a birth certificate by mail

  • Make sure you meet the above requirements.
  • Complete and print the birth certificate application on our Wake County Register of Deeds website at English or Español.
  • Sign the application, attach a copy of the applicant’s valid DMV photo ID or passport, and your $10 check or money order for each birth certificate requested. Do not send cash.
  • Be sure your check or money order is made payable to Wake County Register of Deeds.
  • Mail your application to the Wake County Register of Deeds, PO Box 1897, Raleigh, NC 27602-1897. Your request will be processed within one business day after receiving it. (please consider additional mail time)

Birth certificates from other North Carolina counties

Wake County Register of Deeds does not register or issue birth certificates from other counties, states, or countries. These birth certificates are requested at the place where the birth occurred. Adoption Records

Adoption records are not in the Wake County Register of Deeds Offices. For assistance call NC Vital Records al 919-733-3000.
