Cancer Burden in Africa (1990-2023): A Systematic Analysis of Trends and Risk Factors
Cancer in Africa accounted for an estimated 1·54 million (95% uncertainty interval 1·37–1·75) incident cases, 923 000 (824 000–1 020 000) deaths, and 33·9 million (30·6–37·1) disability-adjusted life-years in 2023, according to a systematic analysis from the Global Burden of Disease Study 2023. The findings reveal that fragmented surveillance data has historically hampered policy responses across all 54 African countries.
The Escalating Health Crisis Across 54 African Nations
The systematic analysis examined fatal and non-fatal outcomes of 48 cancer types across the continent between 1990 and 2023. Researchers utilized the Global Burden of Diseases, Injuries, and Risk Factors Study methodology to evaluate risk-attributable burdens. Out of the total disability-adjusted life-years recorded in 2023, 98.2 percent were attributable to years of life lost.
Modifiable risk factors played a role in regional mortality. The study assessed 88 modifiable risk factors via the comparative risk assessment framework, finding they accounted for 306 000 (254 000–363 000) cancer deaths and 10·7 million (8·9–12·9) disability-adjusted life-years in 2023. Between 1990 and 2023, deaths linked directly to these risk factors surged by 216 percent for both sexes combined.
Gender-Specific Cancer Patterns and High-Risk Demographics
Burden distribution varied significantly across gender and age groups. Among females, breast and cervical cancers dominated the landscape in 2023, together accounting for 53.5 percent of the 960 000 (821 000–1 130 000) total female incident cases. Meanwhile, males faced distinct primary threats. Prostate, liver, tracheal, bronchus, lung, and colorectal cancers represented 44.8 percent of all male cancer cases on the continent.
Younger populations absorbed a heavy share of the overall damage. Childhood cancers among individuals aged zero to 19 years reached 104 000 (84 600–130 000) incident cases and 54 700 (47 500–64 500) deaths in 2023. This represented 27.7 percent of global incidence and 38 percent of global deaths for that age bracket. Adolescents and young adults aged 15 to 39 years contributed 26.6 percent of total cancer disability-adjusted life-years in Africa, pointing to a profound loss of productive years.
Childhood and Adolescent Oncology Pressures
Childhood malignancies represent an acute clinical challenge across the continent. Leukaemia, non-Hodgkin lymphoma, brain and central nervous system cancers, and malignant neoplasms of bone and articular cartilage drove these numbers.
Persistent diagnostic delays and limited access to specialized paediatric oncology care heavily influence these outcomes. Medical institutions face severe capacity constraints when attempting to deliver advanced treatments.
Regional Disparities and Strategic Policy Implications
The rising trajectory of age-standardized mortality and disability-adjusted life-year rates conflicts directly with international benchmarks. The findings stand in sharp contrast to the ambitions of the Sustainable Development Goals for 2030 and the African Union Agenda 2063. Researchers emphasize that mitigating this trajectory requires targeted interventions spanning prevention, early diagnosis, and comprehensive treatment.
Funding from entities like the Gates Foundation supports ongoing surveillance, but local execution remains paramount. Regional policy planning relies heavily on verifiable data to allocate scarce medical equipment and establish modern screening protocols.
As health ministries confront these sobering figures, coordinated national and international action remains the primary vehicle for change. Strengthening health systems against rising non-communicable diseases will define public health success in Africa for decades to come.
