2-Year Data: Icotrokinra Shows Durable Efficacy in High-Impact Psoriasis

Icotrokinra Trial Enrolls Patients with Severe Psoriasis

The phase 3 trial enrolled 311 patients aged 12 years and older with moderate to severe psoriasis who exhibited significant disease involvement in at least one high-impact region. Investigators randomized participants in a 2:1 ratio to receive either 200 mg of oral icotrokinra once daily or a placebo during an initial 16-week blinded phase. Patients in the placebo arm crossed over to active treatment at week 16.

Baseline severity required strict minimum thresholds across targeted regions. These included a scalp-specific Investigator’s Global Assessment score of 3 or higher, a static Physician’s Global Assessment of genitalia score of 3 or higher, a hands and feet Physician’s Global Assessment score of 3 or higher, and a modified Nail Psoriasis Severity Index greater than zero.

Two-Year Efficacy Across High-Impact Anatomical Zones

At the conclusion of the initial 16-week blinded period, 57% of patients receiving icotrokinra achieved an overall Investigator’s Global Assessment score of 0 or 1, signifying clear or almost clear skin, compared to 6% of the placebo group. Following the crossover, response rates among former placebo recipients converged with those on continuous therapy. By week 112, an Investigator’s Global Assessment score of 0 or 1 was recorded in 70% of patients initially randomized to icotrokinra and 64% of those who switched from placebo. Complete clearance, represented by an Investigator’s Global Assessment score of 0, was achieved by 48% of participants in both cohorts.

Region-specific evaluations demonstrated varying timelines for peak therapeutic response. For scalp psoriasis, the proportion of patients achieving a score of 0 or 1 on the scalp-specific Investigator’s Global Assessment rose from 66% at week 16 to 74% at week 112 in the continuous treatment group, and from 11% to 77% in the crossover cohort. Peak responses in the scalp and genital regions manifested around week 24 and persisted without decline through week 112.

Genital psoriasis outcomes followed a similar trajectory. The proportion of patients achieving a score of 0 or 1 on the static Physician’s Global Assessment of genitalia increased from 77% at week 16 to 89% at week 112 among those initiated on icotrokinra, and climbed from 21% to 93% among participants switched from placebo.

Involvement of the hands and feet required a longer duration to reach maximum therapeutic benefit. Peak hand and foot response rates were not observed until week 52, at which point clinical gains stabilized through week 112. Hand and foot Physician’s Global Assessment scores of 0 or 1 climbed from 42% at week 16 to 68% at week 112 in the continuous icotrokinra arm, and from 26% to 66% in the crossover arm.

Icotrokinra Improves Nail Psoriasis over 112 Weeks

Nail psoriasis demonstrated a gradual improvement pattern distinct from other anatomical domains. Mean percentage improvements in modified Nail Psoriasis Severity Index scores reached 33% for the icotrokinra group and 9% for the placebo group at week 16. By week 112, improvements advanced to 71% in the continuous treatment arm and 74% in the crossover cohort, with complete nail clearance achieved in 42% and 54% of patients, respectively.

Richard B. Warren, professor of dermatology and therapeutics at the University of Manchester, noted that nail improvement continued steadily between weeks 52 and 112. Adverse event rates during the blinded phase remained low, with serious adverse events occurring in fewer than 1% of the icotrokinra group compared to 2% of the placebo cohort. Long-term follow-up through 112 weeks revealed no notable differences in overall adverse events or immune function-related complications, including infections.
