Mpox Global situation - Update as of September 19,2025
Overview
This report, the 58th situation report on the multi-country mpox outbreak, details the global epidemiological status of mpox as of August 31,2025,with African-specific data current through September 14,2025,and operational response updates as of September 19,2025.
Key Developments
Despite the recent determination by the WHO Director-General that the mpox multi-country outbreak no longer constitutes a Public Health Emergency of International Concern (PHEIC) following a review by the Emergency Committee on September 4, 2025, mpox remains a critically important public health concern. The WHO continues to actively monitor and respond to the ongoing situation. While the PHEIC designation has been lifted, sustained vigilance is crucial as mpox continues to circulate, notably within Africa, accounting for over 90% of reported cases. The potential for future outbreaks remains globally. Specific populations – including children, pregnant individuals, and those living with HIV – remain at heightened risk of severe illness and mortality. Maintaining robust surveillance, diagnostic capabilities, community engagement, and response infrastructure is thus essential.
A recent rapid risk assessment, updated on September 2, 2025, assessed the overall global public health risk posed by mpox as moderate.
Global Epidemiological Summary
In August 2025, a total of 3780 confirmed mpox cases where reported from 59 countries across all six WHO regions, resulting in 15 deaths and a case fatality rate of 0.4%. Regional trends varied: cases increased in the Eastern Mediterranean and European regions compared to July 2025, while the African region, the Region of the Americas, South-East Asia Region, and the Western Pacific Region all reported decreases.
Emerging Geographic distribution & Viral Clades
Several countries have reported mpox cases for the first time. Kuwait has identified clade IIb MPXV through genomic sequencing. Japan and Senegal have each reported cases linked to clade Ib MPXV.
Australia and Thailand have reported additional cases of clade Ib MPXV, all linked to international travel. Currently, sustained community transmission of clade Ib MPXV is limited to countries in central and eastern Africa.
African Situation
Active mpox transmission is currently reported in 19 countries across Africa. The predominant clade varies by region:
* West Africa: Primarily reports of clade IIb MPXV.
* Central Africa: Reports of both clade Ia and clade Ib MPXV.
* East Africa: Reports of clade Ib MPXV.
All clades of the monkeypox virus (MPXV) continue to circulate globally, and prompt containment of outbreaks is vital to prevent sustained human-to-human transmission and potential community spread.
