While the vast majority of diabetes cases—exceeding 90%, per SBD data—are classified as type 2, and roughly 500,000 individuals in Brazil manage type 1 diabetes, type 5 presents an entirely separate pathogenesis.
Pedrosa noted that type 5 diabetes affects very lean individuals, commonly under the age of 30, who require a lower overall replacement of insulin. Unlike type 2 diabetes, which typically results from insulin resistance developed over a lifetime and comorbidities such as obesity, type 5 patients do not exhibit insulin resistance. Furthermore, they do not develop ketoacidosis, an emergency that occurs when the lack of insulin prevents glucose from entering the cell. Instead, type 5 diabetes stems from a deficiency in the cells that produce the hormone, a problem that results from nutritional deficiency.
The primary clinical hazard driving this new screening effort is misdiagnosis. Because young, lean patients present with symptoms resembling type 1 diabetes, healthcare providers frequently prescribe standard type 1 insulin protocols. Administering high doses of insulin to patients without adequate insulin need forces cells to absorb glucose and reduces the release of sugar by the liver. This cascade can trigger hypoglycemia, leading to clinical manifestations ranging from confusion and seizures to loss of consciousness, coma, and death.
