Dengue Fever Deaths in Jepara: Children Dominated Statistics, Emergency Response Declared

REPUBLIKA.CO.ID, JEPARA — The Jepara Regency Health Service (Dinkes), Central Java, reported that cases of deaths due to Dengue Hemorrhagic Fever (DHF) which occurred from early January to February 29 2024 were dominated by children. Daily Executive 9Plh) Head of the Jepara Regency Health Office, Eko Cahyo Puspeno, said that based on the interim report according to DBD electronically, from January 1 to February 29 2024, there were 15 people who died.

“There are 11 cases in the child category, the rest are teenagers and elderly,” he said in Jepara, Friday (1/3/2024).

For the 11 cases, the age of the children who died was from 2-11 years old. Then two cases were aged 16 and 19 years, while the other two cases were aged 50 and 67 years. Meanwhile, for the total number of dengue fever cases in Jepara Regency, he said, there were 824 cases recorded with details of 689 cases being suspect, 120 positive cases and 15 deaths.

He said, to deal with the spread of dengue cases, the Jepara Regency Government has declared an emergency response status for dengue cases. With the emergency response status established on February 23 2024, it is hoped that the awareness and role of all parties in preventing and controlling dengue fever will increase, including budget support from the Jepara Regional Government.

The public is also asked to participate in fighting dengue fever through prevention with the Mosquito Nest Eradication (PSN) movement together. The Jepara Regency Government is also promoting the Clean Friday Movement and PSN in all institutions, public facilities, including schools, and community environments.

Dengue hemorrhagic fever (DBD) victims who died in Jombang have increased again. Most recently, a 7 year old girl died while being treated at Jombang Regional Hospital. A total of 6 people have died from dengue fever this month in Jombang.

Director of Jombang Regional Hospital, Dr Ma’murotus Sa’diyah, explained that the girl from Tembelang District arrived on Wednesday (28/2) at around 13.20 WIB. The patient was referred by a doctor from RSI Jombang.

“The DSS (Dengue Shock Syndrome) condition was immediately entered into the pediatric ICU,” he explained to journalists, Thursday (29/2/2024).

In less than 24 hours in the children’s ICU at Jombang Regional Hospital, continued Dr. Sa’diyah, the 7 year old boy died. To be precise, this morning at around 04.45 WIB.

“We have confirmed with the doctor who treated him that the patient indeed died of dengue fever,” he explained.

In this way, the number of dengue fever patients who died at the Jombang District Hospital throughout February 2024 became 6 people. Consisting of 5 pediatric patients and 1 adult patient.

Throughout this month, Jombang Regional Hospital treated 182 patients with dengue virus infection (IVD). Consisting of 138 pediatric patients and 44 adults. To date, there are 36 dengue fever patients undergoing treatment at this state-owned hospital. Of that number, 3 patients were treated in the ICU.

Dr Sa’diyah appealed to the public to always adopt a clean and healthy lifestyle. One of them is by eating and drinking more, as well as getting enough rest to increase the body’s immunity.

“Symptoms of dengue fever have changed. Usually after the sixth day it is safe, now it is not, the seventh day is cold, but still critical, people must be alert,” he said.

The case of Dengue Hemorrhagic Fever (DHF) in Cimahi City in early 2024 claimed lives. The victim was a five year old boy.

Head of Disease Prevention and Control (P2P) at the Cimahi City Health Service, Dwihadi Isnalini, said the victim was registered as a resident of Leuwigajah Village, South Cimahi District, Cimahi City.

“It has been confirmed that 1 person has died (due to dengue fever), a 5 year old child in the Leuwigajah area, South Cimahi,” said Dwihadi when met, Wednesday (31/1/2024).

According to the parents’ statement, Dwihadi said that the boy had previously had a high fever. However, he was not taken to any health facility or hospital.

“The information was that the child had a high fever, but was only treated at home. Then he recovered and was able to return to his activities,” said Dwihadi.

Not long after that, the boy returned with a fever with a high temperature. Only then did his parents take the child to the ER at Dustira Hospital.

“His condition was already in decline when he was taken to the ER (Dustira Hospital). His platelets were down, but it turns out he didn’t have time to treat him because he died while he was still in the ER,” said Dwihadi.

Meanwhile, dengue fever cases in January 2024 increased drastically compared to 2023. This year, there were 74 dengue fever cases recorded while in the previous year only 30 cases were recorded.

“There was quite a spike in cases this year, in January there were 74 cases. Last year there were only 30 cases. Most of them attacked children aged 4 to 15 years. Only teenagers and adults ranged in age from 16 to 44 years,” said Dwihadi.

Dwihadi asked the public to be aware of the spike in dengue fever cases considering that we have now entered the rainy season for the next few months.

People usually underestimate the eradication of mosquito nests in unexpected places, such as in the yard, objects that hold water, and even the latest discovery, namely water bottles in cemeteries, is a place for mosquitoes to breed.

“Don’t leave paint marks, buckets or anything that can hold water in the open. If possible, throw away the paint marks straight away so they don’t become nests. The point is to be careful,” said Dwihadi.

Mosquito eggs that survive in dry places during the summer finally hatch when they are flooded with water. In laying one egg, the Aedes aegypti mosquito, which causes dengue fever, can lay 100 eggs.

Morphologically, the Aedes aegypti mosquito is black with silvery white stripes on its body, especially on its legs. That is why this mosquito is also known to be black and white striped. These mosquitoes tend to be more active in the morning and evening, although they sometimes bite at night.

The Aedes aegypti mosquito likes to hide in dark and damp places, both inside and outside the house, such as in a pile of clothes hangers behind the door, under the bed, or among the wild grass that grows in the yard. Therefore, make sure to clean your house diligently so that it doesn’t become a nest for mosquitoes.

Apart from that, preventing the transmission of dengue fever must also be done by eradicating places where mosquitoes breed. Containers that hold water are very risky as a place for mosquitoes to lay their eggs.

The 3M principles are properly implemented to prevent dengue fever, namely draining water reservoirs, closing water reservoirs, and recycling items that have the potential to hold water, such as used cans, bottles, or other items.

The cost burden of dengue fever

Awareness of the threat of dengue hemorrhagic fever needs to be increased because cases of dengue hemorrhagic fever in Indonesia are still high. This condition is at least reflected in data from the Ministry of Health for 2021, 2022 and 2023. Consecutively, cases of dengue hemorrhagic fever reported were 73,518 cases, 143,176 cases and 98,071 cases. Meanwhile, deaths reached 705 cases, 1,236 cases and 764 cases.

In a report by the Social Security Administering Body (BPJS) for Health, the cost of treating dengue hemorrhagic fever has almost doubled. In 2021, health costs for dengue hemorrhagic fever will be IDR 600 billion, while in 2022 it is reported to reach IDR 1.2 trillion.

Deputy Minister of Health Dante Saksono Harbuwono said that awareness of dengue fever must continue to be increased. According to him, even though various control efforts have been carried out, the incidence of dengue fever continues to increase when the El Nino phenomenon occurs. Meanwhile, the frequency and severity of El Nino events could increase due to climate change.

“Every time El Nino comes, the climate situation will change. At that time, cases of dengue fever were also reported to have increased. “That is as reported in 2008 and 2016,” he said.

Early examination

For this reason, in addition to strengthening mosquito vector control, it is hoped that the public will become more aware of immediately carrying out examinations when symptoms and signs of dengue fever infection appear. The increasing risk of dengue fever must be accompanied by early detection and appropriate treatment so that cases of worsening and death can be prevented.

The main symptom of dengue hemorrhagic fever is a sudden high fever reaching a temperature of 40 degrees Celsius. Other symptoms that usually occur are headache, chills, weakness, pain behind the eyes, pain in the muscles and joints, and red spots appearing on the skin.

The erratic rain this rainy season has caused the number of dengue hemorrhagic fever sufferers to rise sharply. One of these things was found at Wonosari Regional Hospital, Gunungkidul, DI Yogyakarta, early January 2010.

In critical conditions, the patient’s body temperature will decrease. This condition must be watched out for because it can indicate that the body is experiencing life-threatening dengue shock syndrome. Often, this condition is realized too late so that the patient arrives at the hospital already in serious condition.

In tackling dengue hemorrhagic fever, the role of the community is very large. The community plays a role starting from efforts to maintain environmental cleanliness by eradicating mosquito nests, killing mosquito larvae using larvicide, and quickly recognizing the symptoms of dengue infection so that they can be treated immediately.

According to Maxi, projections for the dengue vaccine to eventually become a national program will depend greatly on the results of the introduction. The dengue vaccine is recommended by the Association of Indonesian Internal Medicine Experts (PAPDI) to be given in two doses from the age of six years to 45 years as a prevention of dengue fever.

The distance between the first and second vaccines is three months. After that, giving a repeat vaccine four years later is not necessary because antibodies are still high.

Then, talking about efforts to prevent the spread of dengue fever, one of the things the government is trying to do is distribute Aedes aegypti mosquito larvae containing Wolbachia bacteria. Maxi said that currently a number of facilities have the capacity to produce this technology, including the UGM laboratory, Labkesmas in Salatiga.

“Five districts/cities (as a pilot project for implementing Aedes aegyepti mosquito technology with Wolbchia), we will look at it again once this is underway, this year, because our egg production capacity is still limited,” said Maxi.
