Data Review Finds Checkpoint Inhibitors Do Not Improve Survival in LS-SCLC

October 21, 2026 Dr. Michael Lee – Health Editor Health

Adding immune checkpoint inhibitors to standard chemoradiotherapy in limited-stage small cell lung cancer does not significantly improve overall survival or progression-free survival across four major clinical trials, according to data reviewed by CancerNetwork.

Key Clinical Takeaways:

  • Consolidation immunotherapy following chemoradiotherapy showed a pooled overall survival hazard ratio of 0.87, while concurrent administration showed a hazard ratio of 1.11.
  • Severe grade 3 to 4 adverse events varied significantly by trial, ranging from comparable rates in ADRIATIC to an absolute excess of 46.2 percent in the STIMULI trial.

Contrasting Trial Outcomes Across Four Studies

The pooled overall survival hazard ratio across the trials stood at 0.95 with a 95% confidence interval ranging from 0.79 to 1.15, while the pooled progression-free survival hazard ratio was 0.89 with a 95% confidence interval from 0.76 to 1.03. Neither metric reached statistical significance.

Individual trials yielded sharply divergent results. The STIMULI trial reported no overall survival difference at a median follow-up of 22.4 months, showing a hazard ratio of 0.95. Similarly, the ACHILES trial showed no overall survival benefit at 45.1 months median follow-up, recording a median of 43.3 months for the atezolizumab arm versus 38.8 months for controls, with a hazard ratio of 1.14.

Divergence Between Consolidation and Concurrent Strategies

In direct contrast, the concurrent strategy evaluated in NRG-LU005 reported an overall survival hazard ratio of 1.11. Although the test for subgroup differences did not reach statistical significance at a p-value of 0.14, the divergence aligns with the biological hypothesis that administering immune checkpoint inhibitors simultaneously with thoracic radiation may be immunologically counterproductive.

Thoracic radiation causes profound lymphocyte depletion, which can eliminate the effector T-cell populations that checkpoint inhibitors require to function. Concurrent delivery creates temporal competition between cytotoxic cell kill and immune priming, whereas consolidation therapy allows the immune system to recover after radiation is complete.

Durvalumab Shows Survival Benefit over Atezolizumab

Among consolidation trials using anti–PD-L1 agents, durvalumab administered for 24 months in the ADRIATIC trial showed a significant overall survival benefit with a hazard ratio of 0.73. Atezolizumab administered for 12 months in the ACHILES trial did not show a significant benefit, reporting a hazard ratio of 1.14. Beyond structural differences between durvalumab as a fully human IgG1 and atezolizumab as a humanized IgG1, the treatment duration in ADRIATIC was twice as long as in ACHILES.

The dual immunotherapy regimen tested in the STIMULI trial, which paired nivolumab with ipilimumab, suffered from high toxicity-driven discontinuation rates. Approximately 46 percent of patients received fewer than four planned induction cycles, leaving median time on immunotherapy under two months. This abbreviated exposure likely prevented the generation of durable antitumor immunity.

STIMULI Trial Records Highest Immunotherapy Toxicity

Severe adverse events of grade 3 to 4 did not increase significantly across the pooled cohort, yielding a relative risk of 1.08. However, individual trials experienced distinct safety burdens. In ADRIATIC, grade 3 to 4 adverse events were comparable between the durvalumab arm at 24.4 percent and the placebo arm at 24.2 percent. In ACHILES, toxicity was higher in the atezolizumab arm at 34.1 percent compared with 20.0 percent for controls, alongside three treatment-related deaths. STIMULI recorded the highest immunotherapy-attributable toxicity, showing a grade 3 to 4 rate of 46.2 percent compared with 30.7 percent for controls, alongside an adverse-event discontinuation rate of 17.9 percent.

Radiation fractionation schedules also influenced patient outcomes independently of immunotherapy. In NRG-LU005, overall survival was longer with twice-daily radiation than with once-daily schedules regardless of whether patients received atezolizumab, showing median survival figures of 35 months versus 28 months.

Additional context from external reporting by Naver highlights ongoing parallel investigations into combination protocols involving interleukins and monoclonal antibodies, while reviews published by Dovepress detail broader clinical data on combining checkpoint inhibitors with alkylating agents and histone deacetylase inhibitors in oncology.
