Daily Walking as Effective as Intense Exercise for Asthma Control

Trial Design and Patient Cohort Selection
Out of 480 patients screened for the clinical investigation, 52 individuals met the strict inclusion criteria and joined the program. Eligible participants presented with diagnosed moderate-to-severe uncontrolled asthma, maintained stable medical therapy for at least six months without exacerbations in the preceding 30 days, were non-smokers, and were classified as physically inactive by logging fewer than 150 minutes of moderate-to-vigorous physical activity per week. Before randomization, all participants attended an educational seminar covering asthma pathophysiology, correct medication use, and the physiological advantages of exercise.
Participants were subsequently divided via randomization into two distinct intervention arms lasting eight weeks. The first group undertook supervised aerobic training twice weekly at the Hospital das Clínicas, completing 45-minute sessions on a treadmill. The second group attended counseling sessions at the clinic once weekly for up to 90 minutes, receiving behavioral guidance, motivational support, and weekly step-count targets alongside a smart watch for activity tracking. Assessments occurred at baseline, immediately following the eight-week intervention, and during a four-month follow-up.
During evaluation phases spanning seven consecutive days and nights, participants wore a waist accelerometer during waking hours to quantify step volumes and a wrist accelerometer overnight to monitor sleep quality.
Therapeutic Adherence and Clinical Implications
Both non-pharmacological strategies successfully alleviated respiratory symptoms, reduced depression and anxiety scores, and sustained medium-term asthma stability. However, investigators underscored that behavioral and structured exercise protocols function exclusively as adjunct therapies. “We can never stop making use of the treatment that the doctor prescribes,” Pinheiro stated, according to Agência FAPESP.
