ASP-1929 and Pembrolizumab Show 28.2-Month Median OS in HNSCC
ESMO 2026 Presentation Details Long-Term ASP-1929 Survival Metrics in Madrid
Four of five complete responders in a Phase 1b/2 combination study remain alive more than five years after treatment with ASP-1929 photoimmunotherapy and pembrolizumab, according to data presented at the ESMO 2026 congress in Madrid. That survival signal in recurrent or metastatic head and neck squamous cell carcinoma frames an updated analysis that extended median follow-up to 53.1 months across 17 efficacy-evaluable patients.
-
The Tech TL;DR:
- Median overall survival reached 28.2 months in the trial cohort, compared with a historical eight-month baseline for pembrolizumab monotherapy.
- Roughly one in three patients (35.3%) remained alive at four years following treatment with the antibody-dye conjugate and red light activation.
- Rakuten Medical is targeting a U.S. BLA submission in 2028, contingent on the ongoing global Phase 3 ASP-1929-381 trial.
Clinical Context and Historical Survival Gaps in Head and Neck Cancer
Pembrolizumab monotherapy has historically produced a median overall survival of approximately eight months in patients with recurrent or metastatic head and neck squamous cell carcinoma. Against that historical benchmark, the updated ASP-1929-181 study data showed a median overall survival of 28.2 months. Rakuten Medical’s experimental therapy combines ASP-1929, an antibody-dye conjugate targeting epidermal growth factor receptor (EGFR) on cancer cells, with non-thermal red light at 690 nm to induce selective cell necrosis and immune activation. Among the 17 efficacy-evaluable patients out of 19 enrolled, median duration of response reached 44.3 months at the data cutoff. Four of the five complete responders required no subsequent anticancer therapy after stopping study treatment. Exploratory subgroup analyses by HPV status, PD-L1 CPS expression, and tumor size showed consistent results without generating new safety signals, though Rakuten Medical acknowledged that thin subgroup numbers make those findings suggestive rather than definitive.
Global Phase 3 Trial Deployment and Future Regulatory Milestones
The transition from early-phase trials to confirmatory data hinges on the ongoing global Phase 3 ASP-1929-381 study registered under NCT06699212, which aims to enroll roughly 400 to 412 patients. Unlike the earlier recurrent or metastatic cohort, the Phase 3 trial tests the combination as a first-line therapy for locoregionally recurrent head and neck squamous cell carcinoma, a narrower patient population. That shift in patient selection dictates whether the upcoming readout can reproduce the durability observed in the Phase 1b/2 evaluation. Conditional marketing approval has been active in Japan since September 2020 under the conditional framework for cetuximab sarotalocan, but investigational status remains the standard elsewhere. BLA submission is targeted for 2028 depending on the Phase 3 study’s success.
