Randomized trials tie this specific lipid elevation to cafestol, a chemical compound absent in paper-filtered coffee.

Arrhythmia Effects Across Doses and Sources

Cardia rhythm outcomes diverge sharply between moderate coffee consumption and high-dose caffeine products. Data from the DECAF randomized clinical trial evaluated 200 coffee drinkers who underwent successful cardioversion for atrial fibrillation. Participants assigned to drink an average of one cup of caffeinated coffee per day experienced a 47% recurrence rate of atrial fibrillation or atrial flutter over six months. By comparison, the abstinence group experienced a 64% recurrence rate (hazard ratio, 0.61; 95% CI, 0.42-0.89; P = .01).

At the opposite end of the spectrum, high-dose caffeine is linked to malignant ventricular arrhythmias. Energy drinks present a distinct hazard because energy shots pack between 40 and 69 milligrams of caffeine per fluid ounce—three to four times the concentration found in regular brewed coffee. These concentrated beverages have triggered abnormal heart rhythms in otherwise low-risk populations, including healthy adults under the age of 30. Gregory M. Marcus and his co-investigators emphasized that data derived from standard coffee consumption must not be extrapolated to energy drinks.

American Heart Association Excludes Caffeine from 2026 Dietary Guidance

Because rigorous clinical data regarding individualized dosing remain insufficient, the American Heart Association excluded caffeine from its official 2026 dietary guidance. Marcus noted that patient responses depend heavily on age, concurrent medications, underlying comorbidities, genetics, and metabolism. “It’s important to remember that there is no ‘one-size-fits-all’ strategy for safe caffeine consumption,” Marcus stated in the release.
