Xylitol Sweetener Linked to Increased Risk of Heart Attack and Stroke
High blood levels of the artificial sweetener xylitol are associated with a 57% greater risk of major adverse cardiovascular events, including heart attacks and strokes, according to a major longitudinal study presented at the European Society of Cardiology annual congress in Munich.

Researchers compared the 25% of participants with the highest xylitol blood concentrations against the 25% with the lowest concentrations. Within a six-year tracking window, those possessing the highest amounts faced a 57% increased likelihood of death, heart attack, or stroke. Even out to 30 years in extended cohorts, elevated risk persisted at an 18% increase.
According to Dr. Marco Witkowski from the Charité university hospital in Berlin, the findings build directly on prior investigations showing that xylitol can enhance platelet reactivity and promote clot formation. “Artificial sweeteners are consumed by people who think they are healthier than sugar and they are generally regarded as safe by regulatory agencies,” Witkowski stated, pointing out how little contemporary medicine understands about the long-term vascular impact of these food additives.
Prevalence of Xylitol in Consumer Products
Xylitol is a sugar alcohol naturally present in minor quantities within certain foods, but it is manufactured and added—often in concentrations up to 1,000 times higher—to commercially available items. Common consumer products containing the additive include chewing gum, toothpaste, jam, yogurts, and ice cream.
Prof. Dan Atar, an expert member of the European Society of Cardiology communication committee who was not involved with the research, noted that while regulatory authorities previously granted favorable safety ratings, the observational data demands a re-examination. “In this observational work, however, it appears that xylitol has a negative long-term effect on cardiovascular events in the general population,” Atar said. “As with all observational studies, causalities are difficult to infer, but this work shows that further studies are mandated into this societally important topic.”
Dietetic Context and Regulatory Implications
Dell Stanford, a senior dietitian at the British Heart Foundation, emphasized that population-wide efforts to reduce free sugar intake remain critical. “It is important to note that this was an observational study, meaning it can only show an association between higher xylitol levels and cardiovascular events, rather than proving that xylitol directly caused them, and further research is needed,” Stanford explained. “But it is not a green light to have more sugar instead, as we still need to reduce our free sugar intake.”
