Why Pilots and Flight Attendants Face Higher Radiation Cancer Risks
Pilots and flight attendants face a higher rate of radiation-related cancers than workers in more than 500 other U.S. occupations, according to a study published in JAMA Internal Medicine. Researchers examining 12.7 million U.S. deaths from 2020 to 2024 found that flight attendants and pilots ranked highest in deaths from radiation-linked cancers, though public health experts emphasize that frequent flyers face minimal danger.
Understanding the Cosmic Radiation Threat at 30,000 Feet
Commercial jets cruise at altitudes above 30,000 feet, where the Earth’s thinner atmosphere provides less shielding from cosmic radiation. High-energy particles stream into the atmosphere from the sun and deep space. According to the study’s findings, air crew members absorb more radiation on average than any other job classification in the United States, surpassing both nuclear power plant workers and medical professionals handling radiation.
This type of ionizing radiation damages DNA when absorbed in sufficient doses over time. To determine whether career accumulation causes elevated cancer rates, researchers evaluated 12.7 million deaths recorded between 2020 and 2024 across 503 distinct occupations using federal mortality data.
What the Mortality Data Shows Across 503 Occupations
The statistical pattern revealed a distinct alignment with known radiation risks. Flight attendants and pilots accounted for the highest and second-highest shares of deaths from radiation-related cancers—6.9% and 6.7% respectively—after adjusting for age, sex, and other demographic variables. These proportions outranked nuclear technologists, while ground-based aviation workers such as mechanics and assemblers showed no such elevation.
For cancers not linked to radiation, such as colon cancer, air crew mortality rates sat near the middle of the occupational pack. According to Irina Mordukhovich, a research associate at Harvard University’s T.H. Chan School of Public Health speaking on broader cabin crew health studies via Live Science, female flight attendants also experience elevated rates of breast cancer, melanoma, and nonmelanoma skin cancer compared to the general population.
Why Frequent Flyers Do Not Need to Panic
Despite the occupational risks documented for career aviation workers, health experts maintain that casual travelers face negligible threat. Pilots and flight attendants accrue an estimated 3 to 6 millisieverts of cosmic radiation annually over decades of service. By comparison, a passenger taking 10 cross-country round-trip flights in a year absorbs roughly 0.4 millisieverts—a fraction of annual occupational exposure.

Beyond cosmic rays, researchers note that flight crews face compounding occupational hazards, including ultraviolet radiation exposure, disrupted circadian rhythms from jet lag, and historical chemical exposures such as secondhand smoke prior to smoking bans.
Regulatory Gaps and Workplace Protections
While the Federal Aviation Administration recognizes aircrew as occupationally exposed to radiation, U.S. pilots and flight attendants are subject to no federal dose limits and no requirement that their exposure even be monitored. European Union aviation regulators routinely monitor and adjust work schedules to manage carcinogen exposure for cabin crews.
