DELFT – A recent free blood pressure campaign at the Sterflat on Diepenbrockstraat has considerably increased participation in joint consultation hours offered by healthcare, housing, adn welfare professionals, according to Careyn, a care association involved in the “Gezond en Wel Thuis” (Healthy and Well at Home) initiative. the programme aims to help residents over 55 live comfortably in their own homes for longer by integrating care, welfare, support, and living.
Care Manager Ilse Ballegooij-van Gerven of Careyn highlighted the campaign’s success: “Residents can go to the Sterflat with questions about health, well-being or living… The consultation hours recently received a nice boost thanks to an initiative by Careyn!”
Community nurse Bianca Poldervaart, who organized the campaign, reported strong resident interest. “Many residents dropped by for a measurement, a chat or simply out of curiosity. The barrier to entry was lowered and valuable conversations arose about lifestyle, concerns and what people can do themselves to feel better. I was surprised by the large turnout and the impact of these consultation hours.”
The initiative exemplifies a shift towards preventative care and strengthening resident resilience, Poldervaart explained, noting it moves the focus “from responding to complaints to strengthening health and resilience.”
Gezond en Wel Thuis currently operates at four locations in Delft: Parkzicht in the Hof van Delft, De Prelude and the sterflat in the Buitenhof, and the Stefanna in the Voorhof. More details about the program is available at www.gezondenwel-wsd.nl. The collaboration between Careyn, Delft voor Elkaar, Vidomes, and other partners aims to establish this integrated approach to aging sustainably within the community.
