Why US Sanctions on Cuba’s Biotech Industry are Hindering Global Health Security

The story of how Cuba was systematically blocked in its quest to make its own highly effective vaccines widely available offers crucial lessons.

How can humanity prevent the next pandemic from being as disastrous as this one, in which up to 15 million people have died? Last week, the countries of the World Health Organization met in Geneva to begin discussing a pandemic preparedness agreement. A primary goal is to rapidly develop new cures and vaccines, and the ability to deliver them to everyone on the planet.

While no one yet knows what recommendations the WHO will ultimately make, it is possible to predict one thing it will not recommend: easing US sanctions on Cuba’s national biotech industry, which has the wherewithal to develop cutting-edge vaccines and treatments. and share them with countries for which it is impossible to pay the premium prices of First World pharmaceutical companies.

This is a mistake.

During the covid-19 crisis, the United States had the opportunity to share its vaccine technology with the world, and failure to do so prolonged the pandemic at home and abroad.

In June 2022, a senior Biden Administration official admitted that the omicron variant, which has been responsible for more than 300,000 deaths in the United States and more than 1.5 million worldwide, might not have emerged if the world had been sufficiently vaccinated in 2021.

What is less well known is that Cuba had the same opportunity to help vaccinate the world. The story of how Cuba was systematically blocked in its quest to make its own highly effective vaccines widely available offers crucial lessons.

The most recent chapter of this story began in the summer of 2021. The delta variant was devastating India and making its way around the world. The new vaccines offered hope, but poorer countries couldn’t get them for love or money.

While the United States and Europe donated doses, their efforts were barely enough to solve the global problem. Crucially, these governments were unable to persuade the companies they had financed to share the technologies that could have allowed other countries to make vaccines on their own.

Against this bleak outlook, it was surprising to learn that Cuba had created two effective coronavirus vaccines from scratch and then promised to share their intellectual property around the world.

“We realized that we were not going to have money to buy vaccines for our people, so we had to make our own, and we had to do it in a very short time,” Rolando Pérez Rodríguez, director of Science and Innovation at BioCubaFarma, told us recently. .

In August 2021, one of the BioCubaFarma laboratories also produced a booster. Both demonstrated more than 90% efficacy, on par with leading Western vaccines.

The cost of developing these vaccines was 50 million dollars, according to BioCubaFarma, well below the billions invested by the United States Government and the hundreds of millions invested by Germany in theirs.

Surprisingly, Cuba eventually exported almost as many doses of vaccines as it used at home, supplying Venezuela, Mexico, Vietnam, Syria, Nicaragua, Belarus, and Iran. But while many countries in Africa and South Asia also desperately needed vaccines, they did not take advantage of Cuba’s offer.

To explain why they did not, we must go back to 1962, when the economic embargo came into effect. [bloqueo] American against Cuba. Since then, escalating sanctions, which the United States has tightened through the application of constant political and financial pressure, have isolated Cuba not only from the United States but also from the world. The severe sanctions for violating those US measures have made institutions and governments routinely comply with them.

Cuba could have asked the WHO to certify its vaccines to make it easier for other countries to buy them with international aid. But he could not afford to compromise with the WHO after President Donald Trump not only repealed the soft sanctions reforms introduced by his predecessor, but also designated Cuba a state sponsor of terrorism.

This has meant that, even in countries where it is legal to transact with Cuba, few banks are willing to risk heavy fines and criminal penalties for being perceived as supporting terrorism.

Cuba-US relations they are a live political cable, but the new times demand new measures. The world has changed since 1962. The specter that haunts it today is not communism, but another global health emergency.

There is little indication that the Biden Administration will pressure American pharmaceutical companies to share their medical inventions with the world. But President Biden could take a giant step toward global health security by reversing the Trump Administration’s draconian policies toward Cuba.

If it went further, by allowing new exceptions in the US sanctions regime, then Cuba could continue to develop, and share, innovative vaccines and treatments for the world’s diseases.

More than three years later, it is obvious that the world reacted badly to the appearance of the coronavirus, that lives were lost needlessly. But now is the time to prepare for the next pandemic, to set a course for a more equitable distribution of medical technologies. The old embargo [bloqueo] of the United States is not only harming Cuba. It’s hurting the world.

The Impact of COVID-19 Vaccination on Autoimmune Rheumatic Diseases: Patient Reported Outcomes

Patients with autoimmune rheumatic diseases due to the treatments they receive and immunological alterations constitute a group with a higher risk of presenting infectious complications, including severe COVID-19. Although vaccination seems to be an effective strategy to prevent complications from this infection, a significant percentage of patients have decided not to receive it.[1] One of the most frequently mentioned reasons is the fear that the vaccine will trigger the activity of the underlying disease.

It has been reported that up to 5% of patients have a disease flare that requires a change in immunosuppressive treatment.[2] However, it is important to note that there is no universal definition of an activity flare, so patients may report an exacerbation of symptoms that does not warrant a therapeutic change, but that alters the reported outcomes.

Outbreaks in patients with autoimmune diseases after vaccination

In order to assess the incidence of outbreaks reported by patients after vaccination against COVID-19, a group of researchers conducted an international, cross-sectional study, with data from electronic questionnaires.[3]

Two questionnaires assessed short-term and long-term adverse events after vaccination against COVID-19. In these, information on the disease, demographics on comorbidities, treatment, and outcomes reported by patients, as well as on disease activity and other adverse events, were collected.

3,453 questionnaires from patients with autoimmune rheumatic diseases were included. The median age was 48 years (37 to 59) and 84.8% of the participants were female. Autoimmune rheumatic diseases included: rheumatoid arthritis (32.7%), generalized lupus erythematosus (20.9%), axial spondylitis (9.7%), systemic sclerosis (5.5%), mixed connective tissue disease (2 3%), some overlap syndrome (14.5%) and 14% another pathology.

Thirty-seven percent of the patients received steroid treatment and 60.7% reported active disease, most received the Pfizer/BioNTech (43.2%); 11.3% of patients reported having a disease flare with a median time to flare of 57.5 days (10.7 to 188.0); 14.8% had a flare that required an increase in immunosuppressive drugs. The most frequently reported symptoms included arthritis (61.6%), fatigue (58.8%), and muscle pain (53.4%).

The patients who reported outbreaks of activity more frequently had some comorbidity (51.9% vs. 45.3%; p = 0.013), mental health disorders (39.9% vs. 26.6%; p <0,001) y síndromes de sobreposición de enfermedades reumáticas autoinmunes (36,4% frente a 26,9%; p <0,001), en comparación con aquellos que no reportaron un brote de actividad.

Reported disease outbreaks were decreasing, with 16.9% reported in the second dose, 8.6% in the third, and 6.7% in the fourth. The regression analysis found a higher risk of reporting a flare in patients with overlap (odds ratio [OR]:1.4; 95% CI: 1.1 to 1.7), mental health conditions (OR: 1.7; 95% CI: 1.1 to 2.6), and in those who received the Modern (OR: 1.5; 95% CI: 1.09 to 2.2). The use of mycophenolate mofetil and glucocorticosteroids had a protective effect against disease flare-ups.

The authors mentioned that it is difficult to have a definition of a disease outbreak, so different indicators could alter the incidence reported in different studies. However, this research highlights the outcome from the point of view of the patients. Although a significant percentage presented an outbreak of activity, there is no control group, so this effect cannot be definitively attributed to the vaccine.

Expert opinion

Until now, it is not known why some patients achieve sustained disease remission and how the vaccine alters this state of new homeostasis.

Another interesting point was the relationship between the flare-ups and changes in the mental health of the patients, which could be attributed to a worse perception of the disease due to mental stressors or a lower therapeutic compliance associated with these disorders.[4] It is possible that the use of immunosuppressants, such as mycophenolate or steroids, decreased the immunogenicity of the vaccine and, therefore, its association with disease outbreaks.

This study provides important information on the safety of vaccination against COVID-19 in this group of patients who may be uncertain about the effects of the vaccine on their underlying disease. This is why having a large study, with long-term follow-up and an emphasis on patient-reported outcomes helps guide the conversation with them. Although vaccination is the most recommended in all cases, we must be clear about the risks and adverse events that it can entail. As physicians, it is our duty to provide relevant information to help make joint decisions that support the preferences and priorities of our patients.

The CIGB, creator of the immunogen, confirmed that the increase in antibody titers was four times or more compared to the first dose of the drug in these population groups.

The principal investigator of the studies with this vaccine, Sonia Resik, once pointed out that more than 80% of the adverse events reported in children were mild, and the rest of the indicators were comparable to the data obtained in adults, a fact which confirmed their effectiveness.

The Abdala vaccine, on the other hand, maintains its safety, efficacy and immune response to Covid-19 over time, which confirms its suitability for booster doses.

Their immune response was higher in people already vaccinated with three injections of this drug and in others with Sinopharm (China) and Sputnik (Russia).

The Caribbean island also has the immunogens Soberana 02 and Soberana Plus, from the Finlay Vaccine Institute, to fight the Covid-19 pandemic and two candidate vaccines (Soberana 01 and Mambisa) in clinical trials with important results, according to official sources. .
