Four-Year-old Boy Dies from Strep A Infection During National outbreak
Samar, a four-year-old boy, died on December 12, 2022, at Leicester โRoyal infirmary after developing a cough adn โคbeing diagnosed with a severe Strep A infection. An inquest concluded โthat Samar โขdied of natural causes, specifically sepsis due to invasive group A streptococcal infection.
Samar was initially assessed on December 11, 2022, with a FeverPAIN score of 2. The threshold for administering antibiotics at the time had been lowered to 3 โdue to aโ national increase in โstrep A infections. Basedโฃ on his score and clinical presentation,antibioticsโ were not deemed necessary during that initial visit.
Heโ was subsequently โขre-admitted to the emergency department on December 12th. He was quickly assessed with a โpaediatricโ observation scoreโ of 8โฃ and moved to โฃa high dependency bay. โA children’s registrar assessed โขhim within 13 minutes. Aโฃ chest X-ray revealed abnormalities in the rightโข and middle โขlobes of his lung, and a large fluid collection โon the right side of his chest, leading to a diagnosis of empyema.
While preparations were being made to obtain a paediatric surgeon toโ drain the fluid, Samarโฃ collapsed and cardiac resuscitation was initiated. Seventeen cliniciansโข worked for 45 minutes to try and save him, but were unsuccessful.During resuscitation, โฃdrugs were calculated for a weight of 18kg, despite Samar weighing 26kg. though, doctors and anโ independentโ expert witness, Dr. Fitzsimmons, stated this dosageโ error did not affect the โoutcome, given the volume of medication administered during the prolonged resuscitation attempt.
Dr. Fitzsimmons โconcluded that there were no breaches of duty in the โฃstandard of care providedโ to samar during his admissions on December 11th and 12th. The inquest also heard that even if antibiotics had been administered on the morning โฃof December 12th, they โขwould not have had sufficient timeโ to takeโค effect andโค alter the outcome.
Samar was declaredโฃ dead โคat 3.55pm onโฃ December 12, 2022. Atโค least 16 children in โฃthe UK died from invasive โขStrep A in 2022.
Symptoms of sepsis in babies and young children, โคaccording to the NHS, can include blue, grey, โpale or blotchy skin, and difficulty breathing, or increased sleepiness. Strep โA infections areโ generally more common in children and are usually treatable with antibiotics, though they can rarely lead to serious, invasive complications.
