Medicare Advantage Increasingly Impacts Hospitalโข Inpatient Care
Medicare Advantage (MA) enrollment isโ significantly altering the landscape of inpatient hospital care across the United โฃStates. In 2023,Medicare Advantage enrollees accountedโ for 25% of all inpatient days โnationwide,a figure that demonstrates the growing influence of these plans on healthcare delivery. However, โthe impact of MA isn’t uniform; it varies widely by geographic location and even within individual counties.
A clear correlation exists betweenโ county-level Medicare Advantage penetration and โthe proportion ofโค inpatient days attributed to MA enrollees. Counties with a higher percentage of Medicare beneficiaries enrolled in Medicare Advantage plans also tend to have a greater share of inpatient days covered by those plans. Specifically, counties falling within the top quartile of MA penetration saw Medicare Advantage account forโข 28% of inpatient days in 2023, compared to just 18% in the bottom quartile.[Image of Medicare Advantage Comprised a Higher Share ofโฃ Inpatient Daysโ in Counties โWith Higherโข medicare Advantage Penetrationโฃ in โ2023 -[ImageofMedicareAdvantageComprisedaHigherShareofInpatientDaysinCountiesWithHighermedicareAdvantagePenetrationin2023-

This suggests factors beyond overall enrollment rates are at play. For example, in Allegheny County, Pennsylvania – a county with high MA penetration (73% enrollment) – the percentage of inpatient days attributedโ to MA enrollees ranged from 14% to 59% across different hospitals. Similarly,Cook County,Illinois โค(42% enrollment),experienced a range of 2% to โข38% in MA inpatient share across its hospitals.Severalโ factors likely contribute to these discrepancies. Hospital participation in Medicare Advantage โnetworks, the types โขof services โoffered (and the patient populations they attract), โฃand individual beneficiary โคpreferences regarding convenienceโข and perceived quality all โคplayโฃ a role. Hospitals withโ a larger proportion of MA โคpatients may also be more affected by plan-specific policies such as prior authorization requirements,โค claim denials, and network limitations.[Image of Theโ Share of Inpatient Days Attributed to Medicare Advantage Enrollees Variedโฃ Widely Across Hospitals Within Counties in 2023 -[ImageofTheShareofInpatientDaysAttributedtoMedicareAdvantageEnrolleesVariedWidelyAcrossHospitalsWithinCountiesin2023-
