What is the current status of the ‘Roche/ABL Bio’ bispecific antibody BBB shuttle platform performance?
That is why interest is focused on companies that have an antibody-based transport platform (shuttle) that can pass the blood-brain barrier (BBB) to develop new drugs for diseases degenerative brain. In Korea ABL Bio (298380)Like Roche, it has acquired its own drug-based shuttle platform and is conducting clinical trials for a new drug for Parkinson’s disease.
According to the pharmaceutical and biopharmaceutical industry on the 3rd, Roche has again targeted Alzheimer’s with a double antibody. Of the nine new bispecific antibody drugs approved so far in major countries, eight have indications for blood cancer. Only Roche’s macular degeneration treatment drug ‘Bavismo’ (ingredient name farisimab) has attracted attention for other indications. Alzheimer’s (dementia) is the disease that Roche focused on to expand the base of its double antibodies.
In April last year, Roche announced that interim results from phase 1/2 clinical trials of trontinemab confirmed the presence of beta amyloid plaques in the brains of 5 out of 8 Alzheimer’s patients treated with the drug disappeared after 12 weeks. Trontinemab is a dual antibody that combines a shuttle antibody that targets the BBB and gantanerumab that targets amyloid beta. There were no cases of cerebral edema, a major side effect of existing Alzheimer’s treatments, in patients receiving trontinemab.
According to the industry, it is known that it will take 6 months to 1 year for the new Alzheimer’s drug ‘Lechembe’ (ingredient name: lecanemab) or ‘Donanemab’ at Eli Lilly, a new drug candidate that has completed phase 3 , to remove amyloid beta. plaques Roche hopes that its trontinemab will be able to surpass the effectiveness of existing products, as it can eliminate substances that cause Alzheimer’s in just 4 months. The BBB shuttle platform that made this possible is attention grabbing.
The BBB normally plays a role in protecting unnecessary proteins from entering the brain. However, when treating patients with degenerative brain diseases, the BBB acts as a barrier that prevents the entry of drugs.
To overcome this, Roche designed a brain delivery platform that uses an antibody that binds to ‘human transferrin receptor 1’ as a BBB shuttle. When the BBB shuttle antibody, a component of trontinemab, binds to human transferase receptor 1, it penetrates the BBB membrane and enters the brain. Then, another component, gantarunemab, targets and attacks beta amyloid plaques.
Last November, Roche published the results of a POC study on the efficacy of trontenemab on the US National Institutes of Health (NIH) medical database ‘Pubmed’. According to the study, the company compared trontinemab and gantarunemab in primates. At this time, trontinemab and gantarunemab were given in doses of 10 mg and 20 mg per kg, respectively.
As a result, it was determined that trontinemab given at half the dose was at least 4 times and up to 18 times more in the primate brain than gantarunemab. This proved the way in which trontinemab, which has increased permeability into the brain, can be effective in eliminating amyloid beta.
A researcher in the field of new depression drug development said, “Gantanerumab failed in phase 3 because it did not meet the cognitive function improvement index,” and added, “Roche must have been confident in its effectiveness with that he researched for 20 years from the initial discovery of this substance to clinical trials.” “Yes,” he said. He went on to explain, “Ultimately, we believed that by simply increasing the delivery rate of the brain, we could show an effect in terms of improving cognitive function beyond that Lechembe, and we seem to be reborn as a bispecific antibody called trontinemab.
In Korea, ABL Bio discovered ‘ABL-301’, a new dual-drug candidate for Parkinson’s disease that incorporates its own BBB shuttle platform. The company attracted attention in 2022 by signing a technology transfer deal worth 1.3 trillion won with France’s Sanofi for global development rights for ABL-301. ABL-301 is currently undergoing phase 1 clinical trials in the United States and other countries led by Sanofi, and according to ABL Bio, preliminary clinical trial results are expected to be available in the second half of the year this.
ABL Bio has acquired an antibody targeting ‘insulin-like growth receptor 1’ (IGF1R), which has a high level of expression on the surface of the BBB, using the BBB shuttle. The company calls this BBB shuttle the ‘Grabbody-B’ platform.
According to the results of a preclinical study on primates announced by ABL Bio in 2021, the brain delivery efficiency of ABL-301 via Grabbody-B was analyzed to be approximately 13 times higher than a general monoclonal antibody without BBB shuttle antibody.
An official in the dual antibody new drug development industry said, “There is no data directly comparing Roche’s brain permeability and ABL’s BBB Shuttle platform with the same antibody. However, both companies present preclinical results that increase the delivery rate to the brain more than 10 times,” he said, “How efficacy of Roche’s BBB shuttle in humans was confirmed through interim results phase 1/2 clinical trials for trontinemab, for now, Roche is the leading company in this field.” “You could say we are standing on the front line,” he said.
He continued, “If the BBB shuttle platform proves effective in clinical trials and a new drug in use receives accelerated approval, the platform has the potential to be expanded to all areas of degenerative brain disease.” marketing proof of the platform,” he said.

Seoul National University Hospital analyzed CT scans of 1,480 hip fracture patients and designed a deep learning model to predict short-term risk of re-fracture
Excellent predictive accuracy (AUROC) of 0.74…Outperforms existing fracture prediction tools
[이데일리 이순용 기자]A method to easily predict the risk of re-fracture in hip fracture patients is presented. A deep learning prediction model developed by a domestic research team predicted the short-term risk of hip joint re-fracture within 5 years with high accuracy. It is expected that this will help establish existing management and treatment strategies for hip fractures.
The research team, including Professor Young-gon Kim from the Department of Integrative Medicine at Seoul National University Hospital, Researcher Lee Isaac Kim from the Biomedical Research Institute, and the -Professor Seong-hye Kong from the Department of Endocrinology and Metabolism at Seoul Bundang National University. Hospital, developed a short-term re-fracture risk prediction model based on CT images of 1,480 hip fracture patients and confirmed the accuracy mentioned on the 17th.
It is known that patients with hip fracture are at high risk of re-fracture, and that re-fracture occurs on average 2 to 4.3 years after the first fracture. Therefore, it is important to predict the short-term risk of re-fracture and monitor high-risk groups, but existing fracture prediction tools (FRAX, etc.) have limitations in short-term forecasting, so a new forecasting method is needed.
The research team focused on ‘hip joint CT images’, which can determine muscle and bone composition, to develop a short-term re-fracture risk prediction model. From January 2004 to December 2020, the hip joint CT images of 1,012 patients who visited the hospital with a fracture were reconstructed to create frontal, lateral and cross-sectional images. After extracting the features of each image, we designed an ensemble deep learning model that combines them and expresses the degree of freedom of refracture risk (probability of no refracture) in the form of a survival curve .
In addition, all patients were analyzed and ‘reference values’ were set for each time point that passed after the CT scan. If the risk tolerance is lower than this reference value, the probability of re-break increases. Therefore, by comparing the reference value curve with the patient’s survival curve, the time when the survival curve becomes lower than the reference value curve can be predicted as the time in which the event re-break.
In addition, as a result of testing the performance of 468 hip fracture patients, the prediction accuracy (AUROC) of the ensemble deep learning model that predicted short-term recurrence was high at approximately 0.74 (prediction of re-fracture within 2, 3, and 5 years). Accuracy 0.74, 0.74, 0.73, respectively) The closer the AUROC is to 1, the better the prediction performance.
This was a better performance than the FRAX prediction tool which is based on clinical information and bone density. The accuracy of FRAX for predicting re-fracture within 2, 3, and 5 years was 0.58, 0.64, and 0.70, respectively. Based on these findings, the research team emphasized that the CT-based deep learning prediction model can accurately predict short-term recurrence risk for less than 5 years, and explained that this could be used to establish management and treatment strategies for experienced patients. did a hip fracture.
Professor Kim Young-gon (first author) said, “Using the deep learning model developed by the research team will actively help identify high-risk groups for re-fracture,” and added, “For these high-risk groups, we provide prescriptions for osteoporosis, ongoing monitoring, and early rehabilitation.

[이데일리 이순용 기자] March 5th is Gyeongchip (驚蟄), which means that frogs wake up from their winter sleep. Since ancient times, the frozen water of the Daedong River is said to be released after Rain and Gyeongchip, so it has been considered a time when one can feel spring in full swing. In March, when the spring breeze blows along with Gyeongchip and the warmth of new life sprouts, it is the time to renew the body and mind that had shrunk during the winter and welcome spring in earnest. For this reason, every year in March, when spring begins, the number of people visiting hospitals for health checkups increases.
Mr. A, 40, who set health as his goal this year, also underwent a national health checkup at a nearby hospital in the spring. Ms. A was eligible for cancer screening this year, so she underwent a gastroscopy for the first time in her life. A few days later, she was so shocked to see that the results of a gastroscopy were positive in the test results sheet she received through the mail that she went back to the hospital where she had been tested.
When Mr. A was tested for COVID-19 during the COVID-19 pandemic, he was used to the positive test results that were given to infected people and thought that a positive gastroscopy result had a bad meaning. He had trouble sleeping, wondering if he had cancer. However, after visiting the hospital and consulting, Mr. A was able to reassure himself that he had been mistaken all along.
As with COVID-19, if a pathogen test is conducted to check for infection and the result is higher than a certain level, it is said to be positive for the disease. On the other hand, negative means that there is no reaction or if the level is below a certain level, the test is not infected with the disease.
Blood or urine tests that are basically conducted in the national health checkup are also said to be positive if there is an abnormality, and negative if there is no abnormality. However, the results of stomach and colonoscopy tests are different. When cells in the stomach or colon proliferate abnormally and a lumpy tumor is discovered, a biopsy is performed. If the tissue in question is cancerous, it is expressed as malignant (惡性) using the word 惡, which means bad. On the other hand, if it is not cancerous, it is called benign (良性) using the word yang (良), which means good.
Benign tumors are tumors that do not pose a threat to life, except in a few special cases. However, follow-up observation may be necessary depending on the judgment of the medical staff. On the other hand, malignant tumors are tumors that invade surrounding tissues and endanger life, and can be called cancer.
Positive and negative have different meanings not only in tumors but also in tests to check for the presence of antibodies such as hepatitis B antibodies. If you have antibodies to the virus, you are positive. If you do not have antibodies, you are negative. If you are negative, you do not have antibodies to protect against the disease, so you must get vaccinated.
Daedong Hospital Comprehensive Health Examination Center Manager Kim Yun-mi (family medicine specialist) said, “People often get confused like Mr. A because they usually think that the opposite of negative is positive, but the meaning of negative and positive differs depending on the type and purpose of the test. “He said. She also said, “The national health checkup is a project that contributes to leading a healthy life and improving national health by diagnosing and treating health risk factors and diseases at an early stage, so anyone eligible must participate in the checkup to check their health status and maintain their health.” “I recommend managing it,” he said.
Meanwhile, the health checkups conducted by the government include general health checkups and cancer checkups. General health checkups are conducted once every two years for health insurance workers, heads of local households, household members and dependents aged 20 or older, and medical benefit recipients aged 20 to 64. This year, those born in even-numbered years are eligible, with the exception of non-office workers, who are implemented every year.
Cancer screening is conducted for stomach cancer, breast cancer, colon cancer, liver cancer, and cervical cancer, which are the most common in Korea. For stomach cancer, a gastrointestinal contrast test or gastroscopy is performed every two years for those over 40 years of age, and for colon cancer, a fecal occult blood test is performed every year only for those over 50 years of age. Those who test positive select a colonoscopy or colonic double contrast test. . For liver cancer, liver ultrasound and blood tests are performed every 6 months for those at high risk for liver cancer over the age of 40, once in the first and second half of each year, mammography every 2 years for women over 40, and cervical screening every 2 years for women over 20. You can be screened for breast cancer and cervical cancer through cytology, respectively. In addition, health checkups conducted by the government include health checkups for life transitions with medical benefits, student health checkups, and infant health checkups.
