Exposure to a mild or symptom-free strain of Streptococcus pyogenes can trigger a robust human immune response and naturally build protection, potentially solving a nearly 100-year-old medical mystery regarding how adults develop widespread immunity against the pathogen. According to new research published in Nature Communications by scientists at Griffith University and the Murdoch Children’s Research Institute, subclinical infections play a critical role in priming the immune system long before adulthood.

Unraveling the Mechanisms of Subclinical Immunity

Streptococcus pyogenes, commonly known as Strep A, is a prevalent pathogen responsible for painful conditions such as tonsillitis and impetigo, or school sores, particularly in young children and teenagers. If left untreated, infections can progress to severe post-streptococcal sequelae, including rheumatic fever, rheumatic heart disease, and acute kidney disease. First Nations populations in Australia experience some of the highest reported rates of these chronic conditions globally, according to data highlighted by the National Heart Foundation of Australia. Clinical estimates indicate that between 10 and 20 percent of children across Australia contract Strep A tonsillitis annually, with recurrent episodes frequently necessitating surgical tonsillectomy.

Despite the high frequency of childhood infections, epidemiological observations show that by adulthood, most individuals have acquired immunity to a broad spectrum of Strep A variants. This dynamic presented a clinical paradox. Associate Professor Manisha Pandey, senior author of the study from Griffith’s Institute for Biomedicine and Glycomics, noted that how this protection develops had never been adequately explained because there are more than 250 distinct strains of Strep A, and immunity to one specific variant does not confer cross-protection against others.

To investigate this phenomenon, the research team analyzed biological samples gathered from a controlled human infection trial managed by the Murdoch Children’s Research Institute. In that trial, researchers administered a controlled dose of Strep A calculated to induce tonsillitis in a portion of participants while leaving others entirely asymptomatic. All study participants received subsequent antibiotic treatment regardless of their symptom presentation.

Antibody Response and Protection in Controlled Trials

Analysis of the trial samples revealed that participants who remained asymptomatic developed an immune response comparable to those who experienced clinical illness. Dr Ailin Lepletier, a researcher on the team, explained that up to 30 percent of people carry Strep A in their throats without exhibiting signs of illness. The new data indicate that these silent infections allow the body to learn how to fight the bacteria and retain immunological memory from childhood into adult life.

When tested in a murine model of tonsillitis, antibodies extracted from the asymptomatic participants not only bound strongly to the exposed strain of bacteria but actively killed the Strep A germ. Professor Michael Good AO noted that while this natural mechanism sheds light on human immunology, it is unlikely to provide a direct roadmap for a universal vaccine, as immunizations would theoretically need to target each of the 250 circulating strains individually.

Public Health Funding and Clinical Implications

The research was made possible through approximately $2.79 million in funding provided by the Heart Foundation. Dr Anna Calkin, National Manager of Research at the Heart Foundation, emphasized that investing in foundational studies addressing conditions disproportionately impacting First Nations communities is vital for closing longstanding health disparities and preventing rheumatic heart disease. Additional contributors to the published work included Griffith University Professor Daniel Kolarich alongside MCRI researchers Associate Professor Joshua Osowicki and Professor Andrew Streer.
