Refugees Lose Medicaid Access: Impact of H.R. 1 Budget Bill

As of October 1, refugees and asylum seekers across the United States have lost access to Medicaid under provisions set by H.R. 1, the budget legislation passed by the Republican-led Congress in the summer of 2025. NPR reported that the first wave of Medicaid cuts from the One Big Beautiful Bill Act took effect, stripping coverage from the few immigrant groups who had previously maintained eligibility.

Mustafa Rfat, a professor of social work at the University of Nebraska at Omaha who arrived as a refugee from Iraq in 2011, spent the weeks leading up to the cutoff fielding calls from families. He told NPR that refugees with disabilities are terrorized by the reality that their children are losing healthcare coverage.

Ben D’Avanzo with the National Immigration Law Center pointed out to NPR that these populations will eventually attain green card status and regain eligibility, but the intervening lapse in coverage carries steep clinical consequences.

Consequences of Unmanaged Chronic Conditions

Without ongoing access to outpatient clinical services, vulnerable patients typically resort to deferring preventative care and withholding vital prescriptions. D’Avanzo stated that delaying interventions for chronic diseases inevitably leads to deteriorating health outcomes, which ultimately shifts a heavier financial burden onto public hospital systems and government emergency departments once these individuals regain eligibility.

Rfat reflected on his own arrival in 2011, noting that Medicaid served as a vital safety net while he managed a chronic inflammatory condition, learned English, and dealt with the psychological trauma of displacement.

A small number of states are currently working to establish independent coverage programs to bridge the gap for impacted immigrant groups. However, the vast majority of refugees losing benefits face an immediate transition period without access to subsidized medical management.
