Peceleganan Spray Speeds Healing in Mild Burn Wound Infections

Peceleganan Spray Speeds Healing in Mild Burn Wound Infections

October 13, 2026 Dr. Michael Lee – Health Editor Health

Topical Spray Trial Results Released for Burn Wound Infections

A synthetic antimicrobial peptide spray called peceleganan, or PL-5, outperformed a placebo in healing mild burn wound infections during a phase IIIb clinical trial. Researchers evaluated the 2‰ PL-5 spray across 23 centers in China, tracking 348 adult patients to measure wound closure, bacterial clearance, and safety over a 14-day treatment period.

    Key Clinical Takeaways:

  • On day 15, complete wound healing reached 64.3% in the PL-5 group compared to 43.1% in the placebo group.
  • Minimum inhibitory concentration testing showed PL-5 inhibited nine common wound pathogens at a geometric mean MIC of 9.110 micrograms per milliliter or lower.
  • Blood concentrations remained below the lower limit of quantification, indicating no systemic absorption during treatment.

Trial Enrolls Adults with Mild Burn Infections

The trial enrolled 348 adults aged 18 to 65 years old who had mild burn infections covering no more than 10% of their total body surface area. Test wounds ranged from 25 to 100 square centimeters and returned positive bacterial cultures at baseline. Investigators assigned participants in a 2:1 ratio, placing 232 patients in the PL-5 treatment arm and 116 in the placebo arm. Participants applied the topical spray or placebo once daily for 14 days.

Led by researchers at the First Affiliated Hospital of Shenzhen University/Shenzhen Second People’s Hospital, the study was published on October 6, 2026, in Burns & Trauma. The primary endpoint measured complete wound healing on day 15, yielding an absolute risk difference of 21.2% and a number needed to treat of 5.

PL-5 Shows Higher Efficacy than Placebo

By day 15, microbiological efficacy reached 72.3% for the PL-5 group versus 50.5% for the placebo group. Comprehensive efficacy figures stood at 63.1% and 41.9% respectively. Differences between the groups on day 8 were not statistically significant, but by day 21, healing rates progressed to 73.5% for PL-5 and 56.9% for placebo.

Laboratory testing demonstrated that PL-5 was active against common pathogens such as Staphylococcus epidermidis and Staphylococcus aureus. Pharmacokinetic sampling confirmed that blood concentrations stayed below the lower limit of quantification, showing a lack of systemic absorption. Adverse events occurred at similar rates between both groups, with zero drug-related serious adverse events and no deaths reported.

Next Steps for Investigational Peptide Treatment

The study authors noted that PL-5 relies on a membrane-disrupting mechanism designed to lower the likelihood of antimicrobial resistance compared to conventional topical antibiotics. They cautioned that the trial focused exclusively on mild infections, excluded patients with certain comorbidities, and followed participants for only 15 days after treatment ended. Confirmatory studies and regulatory reviews will determine whether the investigational therapy eventually enters standard clinical care for burn wounds.
