Only 13% of Medical Billing Disputes Result in Patient Refunds in Korea
Between 2023 and August 2026, patients in South Korea successfully recovered only 13.4% of improperly billed non-benefit medical expenses through the Health Insurance Review and Assessment Service, according to data released on September 27, 2026, by Democratic Party Representative So Byeong-hun from the National Assembly Health and Welfare Committee.
Patients frequently pay out-of-pocket costs for treatments that legally qualify for National Health Insurance coverage, yet institutional mechanisms fall short of returning the capital efficiently.
Adjudication Volume and Refund Yields
Financial exposure for patients remains high despite procedural interventions by regulatory bodies. Yearly data underscores a consistent gap between contested billing and actual financial restitution. In 2023, the Health Insurance Review and Assessment Service processed claims resulting in 4,123 refunds valued at 15억4,800만원, representing a refund rate of 59.5% among eligible disputed categories. By 2024, refund cases ticked upward to 4,958 instances worth 27억3,200만원, maintaining an allocation ratio of 61.2%. The following year saw 4,241 successful refund actions totaling 17억7,400만원, or 60.3% of evaluated improper billing types. Through August of the current 2026 fiscal year, regulators recorded 2,597 refunds amounting to 7억7,800만원, yielding a 59.6% type occurrence rate.
Total refund expenditures across the entire tracking window reached 68억3,200만원. Multiple refund types can emerge from a single adjudication case, driving a wedge between total resolved complaints and categorized billing errors. Out of 1만8,877 distinct refund violation types cataloged by the Health Insurance Review and Assessment Service, improper non-benefit classification of fully covered baseline care accounted for 52.7% of all occurrences. Non-benefit billing for items ineligible for separate fee calculations constituted another 7.6%. Combined, these two administrative errors represent 60.3% of all registered refund rationales.
Institutional Repeat Offenders and Oversight Gaps
One major tertiary hospital, designated as Institution A, ranked first in refund counts in 2023 with 80 cases, second in 2024 with 66 cases, first again in 2025 with 53 cases, and retained the top position through August 2026 with 34 finalized refund decisions.
Representative So Byeong-hun emphasized the structural necessity of aggressive public outreach. Ensuring that patients recover misappropriated capital requires dismantling information asymmetries regarding review rights. Recurring administrative infractions within identical medical facilities necessitate deeper regulatory scrutiny. When individual institutions generate year-over-year billing discrepancies, oversight frameworks must move beyond isolated consumer refunds. Triggering comprehensive field inspections and rigorous post-audit enforcement ensures institutional accountability and protects consumer capital budgets from persistent leakage.
