NHIS Debt Ratio to Approach 100% Within Five Years, Nongmin Shinmun Reports

October 13, 2026 Dr. Michael Lee – Health Editor Health

South Korea’s National Health Insurance Service (NHIS) faces a projected operating deficit of 21.9 trillion won over the next five years, according to a medium-to-long-term financial management plan submitted to National Assembly member Jeon Jin-suk of the Democratic Party of Korea. The forecast indicates that the agency’s debt ratio will exceed its internal management target of 100% by 2030, as reported by the Nongmin Shinmun.

Key Clinical Takeaways:

  • The NHIS expects total operating losses to reach 21.9 trillion won between 2026 and 2030, driven by rising medical benefit payments.
  • The agency’s debt ratio is projected to climb from 71.2% in 2026 to 107.4% by 2030, surpassing the 100% threshold for financial sustainability.
  • Financial strain is exacerbated by the decision to freeze insurance premium rates for 2027, despite a 1.65% average increase in medical service fees paid to healthcare providers.

Projected Operating Deficits Through 2030

Financial modeling within the 2026–2030 management plan highlights a consistent pattern of annual operating losses. The deficit is estimated to peak at 6.9 trillion won in 2026, followed by 3.97 trillion won in 2027 and 4.29 trillion won in 2028. By 2030, the annual deficit is expected to remain at 3.2 trillion won. These figures are calculated based on the assumption that insurance premium rates will increase by 1.49% annually starting in 2027, following a 1.48% increase in 2026.

NHIS Debt Ratio Projects to Reach 107.4%

The structural imbalance between assets and liabilities poses a significant risk to the long-term stability of the health insurance fund. In 2026, the NHIS projects assets of 44 trillion won against 18.3 trillion won in liabilities. By 2030, assets are expected to shrink to 39.35 trillion won, while liabilities are projected to grow to 20.37 trillion won. This shift results in a debt ratio increase from 68.7% to 99.6% for the overall organization, with the health insurance business unit specifically projected to reach a debt ratio of 107.4%.

Impact of Medical Benefit Payments

The primary driver of the deficit is the rising expenditure on insurance benefit payments to medical institutions. Projections indicate that non-financial liabilities—which include benefit payments and reserves for pending medical claims—will increase by approximately 3.2 trillion won between 2026 and 2030. While the government-mandated medical service fee schedule increased by an average of 1.65%, the Ministry of Health and Welfare decided to freeze insurance premium rates during the Health Insurance Policy Deliberation Committee meeting in September, creating a widening gap between revenue and expenditure.

Proposed Financial Mitigation Strategies

To address the projected shortfall, the NHIS has proposed internal measures, including the expansion of workplace-based enrollment and stricter management of dependents, which the agency estimates could secure an additional 6 trillion won by 2030. Further plans involve generating 2.78 trillion won in financial returns through investments in stable financial products and saving 6.66 trillion won over five years by identifying and reducing fraudulent claims from medical institutions. However, internal analysis from the office of Representative Jeon Jin-suk suggests that these measures lack sufficient efficacy to offset the structural deficit, noting that the health insurance fund operates under a different financial framework than the National Pension Service, limiting its ability to rely on large-scale investment returns.
