Giant ICA Aneurysm with Rare Posterior Cerebral Artery Origins: Case Report

Doctors in an unnamed medical center recently reported the rare case of a giant dissecting internal carotid artery aneurysm with unusual origins of the posterior cerebral arteries, according to a case report published by Cureus.

The patient, whose details were not disclosed, presented with a large aneurysm – an abnormal bulge in a blood vessel – affecting the internal carotid artery, a major blood vessel supplying the brain. What distinguished this case was the aneurysm’s size and the atypical way the posterior cerebral arteries, which provide blood to the back of the brain, originated. Specifically, the patient exhibited both middle and fetal-type origins of the posterior cerebral arteries, a configuration rarely seen in medical practice.

Aneurysms can be particularly dangerous if they rupture, leading to hemorrhagic stroke. The report details the anatomical complexity of this specific case, noting the dissection – a tear in the artery wall – within the aneurysm. The presence of both middle and fetal-type posterior cerebral artery origins further complicated the clinical picture, requiring careful consideration during diagnosis and potential treatment planning.

Another Cureus report details a separate case involving a ruptured aneurysm in a patient with complete duplication of the posterior cerebral artery, successfully treated with endovascular coiling, a minimally invasive procedure to block the aneurysm and prevent rupture. This highlights the evolving techniques available for managing complex cerebrovascular conditions.

A third case report, also published in Cureus, described the incidental finding of an intracranial vertebral artery aneurysm in a patient with a ruptured internal carotid-posterior communicating artery aneurysm. This instance underscores the possibility of multiple aneurysms coexisting, even when a patient presents with a primary vascular issue.

A fourth case, documented in Cureus, involved an acute subdural hematoma – bleeding between the brain and its outer covering – resulting from a traumatic aneurysm of the posterior cerebral artery, but without accompanying subarachnoid hemorrhage, a bleed into the space surrounding the brain. This atypical presentation demonstrates the varied ways aneurysms can manifest and the potential for unexpected complications.

The reports do not indicate the patient’s prognosis or long-term outcome, nor do they detail the specific treatment course undertaken in the initial case. Further research is needed to understand the prevalence, optimal management strategies, and potential risks associated with these rare anatomical variations and aneurysm presentations.
