A foreign worker identified by the pseudonym A Guo died in Taiwan three days after being struck on the head by a shuttlecock while playing badminton with friends.

The incident occurred in a badminton hall in Hukou Township, Hsinchu County. While playing, the man was hit in the head by a fast-moving shuttlecock. He did not collapse immediately, but soon experienced a headache and stopped playing to rest. After packing his belongings, he and his companions left the facility. Shortly after exiting, his condition deteriorated and he suffered persistent vomiting. His companions called the emergency number 119 to take him to the hospital. Despite treatment, the man died three days later.

According to initial assessments by doctors, the preliminary evaluation pointed to a spontaneous increase in intracranial pressure leading to cerebral hemorrhage, though physicians noted this was an initial assessment and had not definitively confirmed the shuttlecock impact as the direct cause of death.

High Speed Impacts Can Fracture Thin Skull Bone

Standard badminton rules state that a regulation shuttlecock weighs between 4.74 and 5.5 grams. However, the article noted that momentum depends on both mass and velocity, allowing a lightweight projectile traveling at high speed to generate significant impact force. Dr. Jui-Feng Lin, a neurosurgeon at Mackay Hospital in Taipei, stated in an interview with TVBS that the skull bone near the temples is relatively thin, with the middle meningeal artery running underneath. An impact from a hard, high-speed object across a small surface area in this specific region can result in bone fractures, vascular injury, and brain bleeding.

The Mayo Clinic explains that an epidural haematoma occurs when a blood vessel between the skull bone and the outer surface of the dura mater tears, allowing blood to accumulate and compress brain tissue without necessarily leaking into the brain tissue itself. Internal brain bleeding can happen without visible external wounds or bruising. Injured individuals may remain conscious and able to speak before severe symptoms emerge, meaning that an affected person walking away and gathering their belongings does not rule out danger.

Studies Document Numerous Shuttlecock Eye Injuries

Data regarding badminton-related injuries highlights risks beyond direct head impacts. A review published in 2023 in the journal Injury Prevention examined 19 studies across 12 countries and documented 378 eye injuries related to badminton, with the shuttlecock accounting for 85 percent of cases where the object was identified, while playing doubles and lacking eye protection were cited as contributing factors. Similarly, research from a specialized hospital at Fudan University in China covering 102 patients between 2018 and 2020 showed that 80 were struck by shuttlecocks and 22 by racquets, requiring 75 surgeries, with 63 cases involving impacts from partners and none of the patients wearing sports protective eyewear. A simulation study published in Ophthalmology Science utilizing a 128.7 km/h impact speed demonstrated that force from a shuttlecock could transfer from the front of the cornea to the back of the eye, causing brief deformation and elevated intraocular pressure.

Health 2.0 noted that sports-related head injuries also occur when players slip or fall while evading fast shots, resulting in impacts against the floor, or when sudden twisting causes neck strain or nerve root irritation.

Head Impacts Require Immediate Medical Assessment

Although the cause is still being investigated, badminton players should be aware that even small shuttlecocks can cause severe injuries, particularly if they strike the eye.

If bleeding is occurring inside the skull, the condition can evolve silently and become serious later. Therefore, if the head receives a strong impact, especially when the temporal region bears direct force, one should not rely solely on the feeling of still being conscious to assess severity. If the injured site shows signs of abnormal depression or an irregular shape, it is necessary to go to the hospital for an immediate check.
