Four trials investigated infections across multiple anatomical sites including the lungs, urinary tract, and skin, while two trials enrolled patients suffering from severe infections accompanied by dangerously low blood pressure.

Geographic diversity across the research pool spanned eight European countries alongside clinical sites in India, Spain, China, France, Egypt, and Taiwan. Approximately half of these investigations maintained short-term follow-up periods of up to one month, whereas the remaining half tracked patients for three months. Ten studies reported no for-profit support or operated independently, while one trial declared partial private support involving the delivery of albumin vials. Methodological weaknesses, however, severely constrain these findings. The certainty of evidence across all primary endpoints was rated as low or very low due to poor study design, incomplete follow-up, and inadequate sample sizes lacking pediatric representation.

Contrasting Albumin With Alternative Intravenous Fluids

When stacked against alternative resuscitation fluids such as crystalloids or synthetic colloids, human albumin yields similarly ambiguous outcomes. The review indicates that researchers are very uncertain whether albumin influences mortality or renal protection, or whether it elevates the incidence of serious unwanted events. Furthermore, data regarding septic shock reduction remain inconclusive, and none of the trials systematically captured patient-reported well-being or broader functional recovery metrics. These limitations highlight a persistent clinical challenge in fluid resuscitation protocols for advanced liver disease.

Future research directives must prioritize large-scale, double-blind, randomized controlled trials that blind both participants and investigators to the administered interventions. Experts emphasize that upcoming protocols must also incorporate pediatric cohorts and directly compare distinct formulations of albumin against varied crystalloid and colloid solutions. Maintaining rigorous oversight and individualized risk stratification remains essential for patient safety in this vulnerable population.
