Colorectal Surgery: Antibiotics That Best Prevent Infection | CIDRAP

A new analysis of over 100 randomized clinical trials has identified specific antibiotic regimens that significantly reduce the risk of surgical site infection (SSI) following colorectal surgery. Published today in JAMA Network Open, the research offers a more nuanced understanding of antibiotic prophylaxis than previous studies, focusing on antibiotic classes rather than simply timing and administration routes.

SSIs are a significant complication of colorectal surgery, affecting an estimated 10% to 25% of patients and contributing to prolonged hospital stays and increased antibiotic use. Current international guidelines generally recommend dual-agent antibiotic prophylaxis before incision to minimize SSI risk, but the optimal choice of antibiotic combinations remains debated.

Researchers at McMaster University in Ontario led the systematic review and meta-analysis, examining data from 18,273 patients undergoing elective colorectal surgery across 105 randomized controlled trials. The analysis evaluated 32 distinct antibiotic regimens to determine which were most effective in preventing SSI. Secondary outcomes included 30-day mortality, adverse events, and length of hospital stay.

The study found that, compared to placebo or no antibiotics, several regimens were associated with reduced SSI risk. However, broad-spectrum penicillins and third-generation cephalosporins demonstrated the most substantial reductions, at 74% and 73% respectively. Notably, broad-spectrum penicillins were also linked to a 79% decrease in 30-day mortality.

The researchers reported no significant differences between antibiotic classes regarding length of hospital stay or the incidence of adverse events. This suggests that the choice of antibiotic class can be prioritized for SSI prevention without negatively impacting other patient outcomes.

The authors emphasize that their analysis provides a more clinically relevant perspective than previous reviews, which often focused on the method of administration. “These findings highlight the importance of antibiotic class selection in prophylactic strategies and provide an evidence base to guide future guideline updates and clinical practice,” they wrote.

Recent reports from the Centers for Disease Prevention and Control (CDC) have underscored the growing threat of antibiotic resistance, adding urgency to the need for optimized antibiotic stewardship. Libya recently announced the elimination of a highly contagious bacterial eye infection, a success attributed to targeted public health interventions, according to a report from CIDRAP. This highlights the potential for focused strategies to combat infectious diseases.

A separate study, published by Cureus, examined the comparative outcomes of open versus laparoscopic colon surgery, analyzing postoperative complications, recovery times, and long-term survival. While not directly related to antibiotic prophylaxis, this research underscores the multifaceted approach required to improve patient outcomes following colorectal surgery.

Researchers are also developing predictive models to identify patients at higher risk of nosocomial infections after colon cancer surgery. A dynamic nomogram, recently published by Frontiers, aims to predict these infections, potentially allowing for more targeted preventative measures.
