As Liu noted, gray matter processes information while superficial white matter helps nearby brain regions communicate. The study’s authors utilized an advanced form of diffusion magnetic resonance imaging (MRI) to assess neurite density—the small projections enabling nerve cells to transmit and receive signals—alongside the volume of freely moving water surrounding them.

Tissue disruption marked by lower neurite density or elevated free water often reflects pathological processes such as myelin loss, inflammation, or localized swelling. Within the LASI-DAD cohort of 459 participants aged 60 and older, researchers observed that decreased gray matter remained the strongest overall predictor of cognition. However, this relationship depended significantly on structural integrity. When local white matter wiring showed poorer integrity, gray matter loss tied more strongly to impaired language performance and cognitive deficits. Conversely, healthier superficial white matter appeared to lessen these negative associations.

Clinical Implications and Resilience Pathways

Senior author Leon Aksman, assistant professor of research neurology at the University of Southern California, described superficial white matter as a potential biological source of resilience. According to Aksman, two individuals experiencing a similar degree of gray matter loss may not experience identical cognitive effects if the local connections surrounding that gray matter differ in health. The clearest protective links appeared in frontotemporal areas responsible for word recognition, fluent speech, and holding language information in mind.
