We are seeking a highly motivated Ambulatory Care Clinic Pharmacist to join our team at Cedars-Sinai Medical Center in Los Angeles, California. this is an Advanced Practise Pharmacist role, central to our ambulatory and specialty pharmacy programs, with a specialized focus on neurological conditions and their associated specialty and infusion therapies.
In this role,you will:
* Collaborate with neurologists and interdisciplinary teams to optimize clinical outcomes and improve operational efficiency.
* Provide complete medication management including therapy monitoring, side affect management, and patient education for self-administered and infusion medications.
* Independently manage therapies – initiating, adjusting, and discontinuing medications based on current neurology guidelines (American Academy of Neurology) and cedars-Sinai P&T approved protocols.
* Order and interpret laboratory results and authorize medication refills.
* monitor and report on patient adherence, therapeutic outcomes, and satisfaction levels of both patients and providers.
* Serve as a clinical resource for physicians and support staff.
* Contribute to quality improvement by developing and updating protocols, competency assessments, and initiatives.
