Statewide‌ – ⁣Reinstatement & Terminated⁤ Monitoring

In September 2025,the DOH terminated probation for two individuals: MaLeigha Miller (CO61211020,MC61207378),a ⁤substance use disorder professional trainee and mental health counselor associate,and ⁢ Xandra Lee Abram ⁣(CO61429908),a substance use disorder professional trainee. Additionally, ⁤probation and monitoring were ⁢terminated for veterinarian Susan Carroll ‌Connor (VT00003002).

pierce County

The DOH charged massage therapist Jennifer Diana Ortiz (MA00020960) with unprofessional conduct. The charges allege ⁤Ortiz practiced massage ⁤therapy without a‌ license between June 2020 and April​ 2024.‍ An expired credential activation submission was submitted ⁤in august ‍2024.

Snohomish County

Shari Arvilla ‌Lambertz (NC60078638), a certified nursing⁤ assistant, was charged with unprofessional ⁣conduct for allegedly failing to disclose a 2006 gross-misdemeanor conviction for ​third-degree malicious‌ mischief on her reactivation application.

The DOH⁤ denied ⁣the​ application of Dylan wayne Vanosdol (CG61612282) for an ⁤agency affiliated counselor credential in August 2025. ⁤This denial stems from convictions in 2022 for controlled⁤ substances​ homicide ⁣and violation of the Uniform Controlled Substances Act (both class B felonies), and also second-degree identity theft (class C ⁣felony) and third-degree theft (gross misdemeanor).

Spokane County

The DOH ​issued a ⁣notice of intent to issue ⁣a cease-and-desist order to Corinthia ⁤Ridenbaugh for allegedly practicing massage without ⁤a​ license between 2022 and March 2025, operating ⁢a massage business‌ and providing services without⁣ proper credentials.

social worker⁢ associate Robert ⁣Thomas ⁣Tarver (SA61340852)‍ was charged with unprofessional conduct related⁣ to allegations of coercing a ​client into⁣ sexual contact in 2020. He⁢ was convicted in ​July 2024 of ⁤receiving unlawful compensation with sexual motivation (class C felony) and first-degree perjury ‍(class B felony).

Whatcom County

The DOH⁢ charged certified nursing assistant Nancy J.Kromer (NC60044203) with unprofessional conduct, alleging she⁣ neglected a⁢ vulnerable adult under her care and ⁣engaged in‌ an inappropriate relationship with the client’s ⁣spouse between August and⁢ October 2023.

Out of State ​- Colorado

The DOH reinstated the registered nursing assistant credential of Jessica‌ LynnMarie Younkerman ​ (NA60916601) in Colorado in September 2025, subject to regulatory⁢ requirements.

Meaningful Note: The DOH states that healthcare​ providers charged with unprofessional conduct have 20 ‌days to⁣ respond in⁣ writing. Cases then⁣ enter a settlement process, and if no agreement​ is⁤ reached, will proceed to ​a hearing.
