Oklahoma Woman Charged with Practicing Dental Hygiene with Forged Licenses
YUKON, OK – A Canadian County woman has been formally charged with multiple offenses related to allegedly practicing dental hygiene without a valid license and using forged documents. Christy perkowski,46,faces criminal charges following an investigation initiated by the Oklahoma Board of Dentistry.
According to an affidavit of probable cause filed with the District Court of Canadian County, Perkowski is accused of working as a dental hygienist at a Yukon dental clinic for over four years using falsified credentials. She has been charged with five counts of practicing dental hygiene without a license and two counts of uttering a forged instrument. A conviction on all counts could result in a prison sentence exceeding one year, along with associated fines.
Details of the Allegations
The Oklahoma Board of Dentistry received notification in April of this year regarding the alleged use of a forged license by Perkowski. The affidavit states that between january 2001 and April 2025, Perkowski allegedly provided dental services to patients, including preventative treatments and the governance of local anesthesia and nitrous oxide.
Records indicate Perkowski’s original dental hygienist license expired in 2020. To continue employment at the Yukon clinic, she is alleged to have presented forged Oklahoma Board of Dentistry registered dental hygienist licenses dated 2022 and 2024 to clinic staff.
