2027 TRICARE Health Plan Costs and Coverage Changes

November 5, 2026 Dr. Michael Lee – Health Editor Health

Active-duty family members, retirees, and reservists will see shifts in their healthcare costs driven by federal cost-of-living adjustments, statutory changes, and shifting medical and prescription service expenses. These adjustments, effective Jan. 1, 2027, encompass changes to enrollment fees, premiums, cost-shares, and copayments.

    Key Clinical Takeaways:

  • TRICARE Open Season runs from Nov. 9 to Dec. 8, allowing eligible beneficiaries to modify their Prime or Select coverage for the upcoming year.
  • Active-duty service members maintain zero out-of-pocket costs, while family members and retirees experience specific adjustments based on whether they fall into Group A (sponsor enlisted before Jan. 1, 2018) or Group B (sponsor enlisted on or after Jan. 1, 2018).
  • Monthly premiums for premium-based plans such as TRICARE Reserve Select and TRICARE Retired Reserve increase across the board for 2027.

Open Season Windows and Beneficiary Group Classifications

Decisions made during the Open Season window, which closes on Dec. 8, determine coverage parameters for the entirety of 2027. Debra Fisher, health system specialist at the TRICARE Health Plan within the Defense Health Agency, noted that reviewing these projected expenses allows beneficiaries to evaluate whether their current enrollment suits their household needs.

Beneficiary categories divide into two distinct cohorts. Group A includes individuals whose sponsor initiated service before Jan. 1, 2018, whereas Group B applies to those whose sponsor’s initial enlistment or appointment occurred on or after that date.

Active Duty Family Member Adjustments

For active duty family members, costs are contingent upon the sponsor’s pay grade, the specific TRICARE plan, and the location of care. While these beneficiaries generally face no copayments, costs are incurred if plan rules are not followed or if prescriptions are filled outside of military pharmacies. Group B members face higher deductibles, rising to $68 for individuals and $137 for families (E-4 and below) or $205 and $411 (E-5 and above).

Retirees Face Higher Enrollment Fees and Premiums

Retirees and their families encounter separate fee schedules. TRICARE Prime annual enrollment fees for retirees move to $396 for an individual and $792.96 for a family in Group A, compared to $480 and $960 for Group B. TRICARE Select annual enrollment fees for retirees land at $193.92 for an individual and $388.92 for a family in Group A, while Group B retirees pay $616.92 and $1,233.96 respectively.

TRICARE Young Adult premiums to hit nearly $900 a month in 2027

Premium-based plans also adjust upward for 2027. TRICARE Reserve Select, open to Selected Reserve members but excluding those on active duty orders for over 30 days, sees monthly premiums increase from $57.88 to $60.54 for member-only coverage, and from $286.66 to $307.48 for family coverage. TRICARE Retired Reserve, available to retired reservists under age 60 and their survivors, climbs to $707.59 for member-only and $1,697.70 for family coverage, up from 2026 rates of $645.90 and $1,548.30. TRICARE Young Adult, covering unmarried children ages 21 to 26, shifts to $394 for Select and $893 for Prime. Temporary Continued Health Care Benefit Program coverage, which provides 18 to 36 months of transitional care, moves to $2,280 for single participants and $5,741 for families.

Pharmacy Costs Remain Stable While Dental Figures Pend

While TRICARE Pharmacy Program costs remain stable without scheduled changes for 2027, and current TRICARE Dental Program premiums hold steady through Feb. 28, 2027, specific figures for TRICARE For Life remain pending. The Defense Health Agency indicated that TRICARE For Life cost structures for the upcoming year will be published early next year. While these plans include catastrophic caps—the maximum amount a beneficiary pays for covered care annually—the 2027 figures for these caps have not been announced. Premiums do not count toward these caps and must be paid regardless of whether the limit is reached.
