A survey conducted by the Conference of Presidents of Medical Commissions of Hospital Centers (CMECH) from May 27 to June 14 confirms the criticality of the situation in the public hospital.

The situation and outlook are “very disturbing“indicates the conference, following its survey of establishment medical commissions (CME) and group medical commissions (CMG).

Regardless of the metropolitan or overseas region, or the type of establishment (GHT support or not), 63.7% report a major difficulty currently within their establishment and 97.6% anticipate it by to 2023“, says the survey.

The proportion of failures affects emergency structures as much as medical, follow-up care and rehabilitation (SSR) services, accommodation establishments for dependent elderly people (Ehpad), long-term care units (USLD) and psychiatry, she says.

Perinatal, surgery and critical care services are also significantly impacted and city medicine is at the top of the list of concerns, she demonstrates.

Recruitment difficulties

The conference points in particular to the difficulties “majors” recruitment in emergency services, also important in the sectors of perinatal care, medicine, SSR, Ehpad/USLD, psychiatry.

Those relating to the recruitment of paramedics come first in the departments of medicine, surgery, SSR and Ehpad/USLD.

Another observation: bed closures are increasing. 32% of respondents report critical care bed closures; 20.7% in perinatal care, 44% in surgery, 74.1% in medicine and 45.3% in psychiatry.

These bed closures, linked to absenteeism and the departure of professionals, are at the origin of the acceleration of the dysfunctions, which are also chronic, of the emergency services.“, says the conference. In total, several thousand beds are closed in addition to the usual deficiencies and the public hospital has the greatest difficulty in admitting patients who need it to hospitalization;

In addition, the public hospital suffers from major difficulties in getting its patients out when they need rehabilitation or medico-social care structures. Bed closures are also frequent there: 61.7% in SSR and 41.6% in Ehpad/USLD.

This results in deprogramming “always significant“despite the lack of current impact of Covid 19 (32.7% in surgery and 17.1% in medicine).

Urgent measures including increase in staff remuneration

Institutions feel a little isolated in the management of this crisis“, says the survey.


Coordination of bed closures within GHT is only implemented for 42.2% of respondents. The ARS seem weakly involved, according to the survey, with an average evaluation of their mobilization of 3.69 (scale of 0 to 10) for the central directorates, and 3.41 for the territorial delegations. The estimated mobilization of other actors remains “more than timid”: 1.74 for private clinics, 1.98 for ESPICs and 2.63 for city medicine, which itself is facing great difficulties.

Faced with these findings, the presidents of CME and CMG believe that the urgent measures to be taken to respond to the hospital crisis concern the substantial increase in the remuneration of paramedical staff and medical staff. More than half believe that the effort of all revaluations should aim for an overall increase in remuneration of 20 to 30% to be acceptable.

They also ask for the guarantee of recruitment “to improve the balance between the number of healthcare staff and the patient care load“, a second phase of investment in public hospitals, the extension of the funding guarantee, the accentuation of the medicalization of governance and the drastic reduction of the administrative burden.
