Serious Inflammation Related to Covid-19 Also Affects Adults

Multisystem inflammation in Covid positive children is identified as a new syndrome.

This was reported by the Centers for Disease Control and Prevention (CDC) United States (US). Multisystem inflammatory syndrome in children (MIS-C) usually involves shock, heart damage, stomach upset, and hyperinflammation.

The CDC used reports from 27 adult patients to describe a similar new condition, multisystem inflammatory syndrome in adults (MIS-A). Patients reported having cardiovascular, gastrointestinal, dermatological, and neurological symptoms without severe respiratory disease and simultaneously receiving a positive test result for a new type of corona virus (SARS-CoV-2), which causes Covid-19 disease.

Much remains unknown about MIS-A, with much uncertainty over the timeline from infection with the new coronavirus, to the onset of MIS-A. However, the report says MIS-A and MIS-C may represent a post-infection process.

Adults reporting typical symptoms Covid-19 then developed MIS-A about two to five weeks later. Patients with MIS-A may not test positive for the new coronavirus, given the onset of the syndrome a few weeks later.

The CDC emphasizes the importance of antibody testing for previous SARS-CoV-2 infection to recognize and treat MIS-A. Of the 27 MIS-A patients included in the report, 30 percent of them, along with 45 percent of the 440 children with MIS-C reported to the CDC as of July 29 tested negative, but are currently positive for Covid-19.

The criteria used to identify MIS-A include severe disease requiring hospitalization in patients over 21 years of age. In the past 12 weeks, severe dysfunction of one or more organs other than the lungs has been found, laboratory evidence of severe inflammation and a lack of severe respiratory disease.

Patients are mostly treated with corticosteroids among other treatments such as vasopressors to increase blood pressure or blood thinners. The report also noted that all but one of the patients in the report were from a racial or ethnic minority revealing that MIS-C had also been disproportionately reported in that community.

However, because of the small sample size, the CDC says, more research is needed before making any conclusions about the MIS-A burden across groups. The findings suggest that adult patients of any age with current or previous Covid-19 can develop a hyper-inflammatory syndrome that mimics MIS-C.

The CDC added that measures to limit the spread of the outbreak could help prevent MIS-A.
