There is the possibility of doing a semi-medical internship for a period of 16 weeks in the gastro-intestinal-hepatic (MDL) department.

The internship will consist of an 8-week inpatient period and an 8-week outpatient period.

Objective

The semi-medical internship in the department of gastroenterology is particularly suitable for interns with a special interest in gastrointestinal and liver diseases and for those who are considering training to become gastroenterologists. During the internship, the objective will be for the semi-doctor to take care of the patient with an increasing degree of autonomy and responsibility.

clinical period

The semi-doctor will follow the gastrointestinal and liver diseases department. In this department there is a variety of patients covering the entire specialty of gastro-intestinal liver diseases. Since liver transplants are performed in Leiden, patients both before and after liver transplantation represent a substantial portion of the hospitalized population. During the ward apprenticeship, the semi-doctor accompanies the visitors on a daily basis. As a semi-doctor you have a number of your patients (the number depends on the severity of the patients and the range of admissions). You see these patients yourself, in the presence of the doctor(s) on the ward. Decisions regarding medical policy and drug prescribing should be discussed with the departmental clinical supervisor (or, if there is no clinical supervisor, the departmental physician). It is also important that meetings with family members and information meetings with the patient take place in the presence of a supervisor (ward doctor or clinical supervisor). During your clinical placement you will need to attend all meetings mentioned in the weekly schedule below and introduce patients to these meetings yourself.

outpatient internship

The Gastrointestinal and Liver Disease outpatient clinic consists of a series of consultation hours for general Gastrointestinal and Liver disease and a series of consultation hours where you see specialist patient populations. The latter covers IBD surgery, liver disease and liver transplantation, and oncology. During the 8-week outpatient internship, the demi-physician will strive to see at least one new patient each day. The intention is that a complete history and physical examination be taken, after which a problem list, summary, differential diagnosis, plan, and treatment proposal (including medication prescription) are made. This is then discussed with the doctor who will review the patient at a subsequent office visit. The information meeting that will then take place will be conducted by the semi-doctor in the presence of a gastroenterologist. A letter should be written to the contact person for each new outpatient. This letter will be sent to the referent after the co-signature by the gastroenterologist in question.

Attendance at the various transfers, interviews and teaching moments is also a requirement during the outpatient internship.

Endoscopy

Time is allocated for endoscopy monitoring during the inpatient and outpatient period. A fixed part of the outpatient internship consists of autonomously carrying out gastro and colonoscopies and training in endoscopy on an endoscopy simulator (Simbionix). This is done under the supervision of an MDL physician.

Paper

The GI conference takes place every week, where one of the doctors gives a lecture or presentation. In the final weeks of the semi-medical internship, the semi-medical should give a lecture or presentation at this meeting.

Research

The conduct of scientific research will be facilitated wherever possible.

Assessment

The clinical training is evaluated by the chef de clinique (after 4 and 8 weeks).

The final assessment takes place after the outpatient internship, at the end of the 16 weeks.

The internship period

Since the internship consists of two 8-week periods, a new semi-physician can start the internship every 8 weeks and up to 2 semi-physicians can work on diseases of the stomach, intestines and liver at the same time.
