Characteristics of patients who died positive for SARS-CoV-2 infection in Italy

As of October 22, according to ISS, the mean age of SARSCoV-2 positive deceased patients is 80 years (median 82, range 0-109, InterQuartile Range – IQR 74-88). There are 15,719 women (42.7%). Figure 1 shows that the median age of SARS-CoV-2 positive patients who died is 30 years higher than that of infected patients (median age: patients who died 82 years – patients with infection 52 years ). Women who died after contracting SARS-CoV-2 infection are older than men (median age: women 85 – men 79).

In 91.1% of hospitalization diagnoses, conditions (e.g. pneumonia, respiratory failure) or symptoms (e.g., fever, dyspnoea, cough) consistent with COVID-19 were mentioned. In 387 cases (8.9% cases) the diagnosis of hospitalization was not related to infection. In 57 cases the diagnosis of hospitalization concerned exclusively neoplastic pathologies, in 131 cases cardiovascular pathologies (for example acute myocardial infarction-MI, heart failure, stroke), in 52 cases gastrointestinal pathologies (for example cholecystitis, intestinal perforation, intestinal obstruction, cirrhosis) , in 147 cases other pathologies.

Symptoms most commonly seen before admission in patients who died positive for SARS-CoV-2 infection. Fever, wheezing and cough are the most common symptoms. Less frequent are diarrhea and hemoptysis. 7.5% of people had no symptoms at the time of admission.

Respiratory failure was the most commonly observed complication in this sample (94.2% of cases), followed by acute kidney injury (23.3%), superinfection (18.9%) and acute myocardial injury (10.7 %).

Therapy

Antibiotic therapy was commonly used during hospitalization (86.5% of cases), antiviral therapy less used (57.6%), steroid therapy more rarely (45.2%). The common use of antibiotic therapy can be explained by the presence of superinfections or is compatible with the initiation of empiric therapy in patients with pneumonia, awaiting laboratory confirmation of COVID-19. In 1138 cases (26.5%) all three therapies were used. 4.6% of patients who died positive for SARS-CoV-2 infection received Tocilizumab.

Times

The median times (in days) from symptom onset to death (12 days), from symptom onset to hospitalization (5 days) and from hospitalization to death (7 days). The time from hospitalization to death is 6 days longer in those who have been transferred to intensive care than those who have not been transferred (12 days versus 6 days).
