Doctors spoke about the danger of COVID-19 for the children’s heart

COVID-19 can cause in babies cordial failure, even if they are asymptomatic, according to American doctors. Previously, similar complications were recorded mainly in adults.

American doctors from the Children’s Hospital at Montefiore Medical Center are faced with a case of myocardial damage in a two-month-old baby infected with COVID-19. Medical history described in the journal JACC: Case Reports.

Since the first reports of COVID-19, some patients have shown signs of myocardial damage. A closer study of this issue made it possible to find out that this symptom was observed in 20-28% of hospitalized patients. However, most of them were adults and had cardiovascular diseases, while in children, myocardial lesions were practically not studied. In this case, the doctors had to deal with him in a two-month-old child.

The child was born prematurely, at 33 weeks, and spent three weeks in the intensive care unit for children, of which he spent a week on a continuous positive pressure apparatus.

After being discharged, he felt well, but at two months he suddenly began to choke after feeding, his face turned blue.

In appearance, the baby was healthy – he had neither temperature nor cough, no diarrhea, or any other sign of infection. On the way to the hospital, his blood pressure dropped and his heartbeat became too fast.

The first test for COVID-19 was negative, but the second revealed the presence of SARS-CoV-2. Blood tests indicated mild inflammation and liver damage. The main problem became clear after X-ray and ECG: the baby was faced with heart failure and myocardial damage. The reason for this, most likely, was a viral infection.

At the hospital, the child received drugs that correct the work of the cardiovascular system, as well as mechanical ventilation of the lungs. Doctors also used steroids to reduce inflammation and antibiotics to treat bacterial complications. The child’s condition gradually improved, and on the 16th day of treatment, he felt quite well. Later, his parents were allowed to take him home, but doctors continued to monitor his condition.

“This patient’s case, including the treatment regimen, is very similar to four other reports of myocardial injury in adult COVID-19 patients,” said Dr. Madu Sharma, lead author of the study. – Most children with COVID-19 are either asymptomatic or have mild symptoms, but our case shows that babies can also face acute myocardial damage. Testing children with signs and symptoms of heart failure for COVID-19 is very important as we learn more about the effects of this virus on the heart. ”

Doctors at the Texas Children’s Hospital also warned about the danger of coronavirus for children’s hearts.

Children’s multi-system inflammatory syndrome (MIS-C) is an inflammatory process affecting several body systems at the same time, causing children to have a high (38.5 ° C and higher) temperature, low pressure, oxygen deficiency and, in some cases, organ failure, which brings it closer to clinical manifestations with Kawasaki syndrome.

Apparently, the syndrome occurs as a complication of infection with SARS-CoV-2.

Fever and hypotension are most commonly reported, as well as a number of rarer symptoms, including stomach ache and throat, cough, confusion, diarrhea, swelling of the limbs, nausea, vomiting and headache. The examination can reveal damage to the cardiovascular system, such as myocarditis and pericarditis, as well as coronary artery disease.

More than half of the children with MIS-C had heart disease. Many have suffered from heart failure. They experienced dilatation of the coronary vessels, decreased ability of the heart to pump oxygenated blood into the tissues of the body, and almost 10% of children encountered a coronary aneurysm – its localized swelling. In the future, they are more at risk than others. heart diseases.

Also, compared to COVID-19, the levels of inflammatory markers in MIS-C were much higher. For example, troponin, a marker used in adults to diagnose heart attacks, was 50 times higher than normal levels in children with MIS-C.
